We are looking to hire a qualified and compassionateLicensed Vocation Nurse (Floor) for their Primary Care Department. 

Reporting to the Director of Operations, the LVN underthe direction of a physician, advanced practice provider, or Registered Nurse,delivers compassionate patient care and works collaboratively with thehealthcare team to promote health, prevent illness, and support positivepatient outcomes. 

Schedule: Full-Time | 1.0 FTE

What You’ll Do  

Key Responsibilities  

  • Demonstrate the health center’s mission by providing quality care and serving as an LVN resource to the care team, as applicable 
  • Perform nursing duties within scope, including wound care, immunizations, and patient health education 
  • Complete medication reconciliation and administer prescribed medications and treatments per written or standing orders 
  • Maintain up-to-date emergency kits and oxygen tanks 
  • Complete required forms in accordance with health plan and program standards 
  • Performs all duties in support of successful EHR and patient portal management  
  • Contact patients to schedule appointments as needed 
  • Participate in care team and other assigned meetings 
  • Deliver services in full compliance with the health center’s standards 
  • Respond to and appropriately escalate provider and patient messages using SBAR and established health center policies and procedures 
  • Communicate through messages with external providers and agencies regarding patient care, issue resolution, and escalation 
  • Respond to patient portal requests for forms, labs, and test results within scope 
  • Perform other related duties as assigned 

What We Are Looking For  

License, Certification and Experience  

  • High school diploma or equivalent 
  • Current, valid California LVN license 
  • BLS certification required 
  • 1+ year of LVN experience; outpatient clinic experience preferred

  • Ability to work effectively in a fast-paced environment 
  • Excellent written and verbal communication skills

  • Bilingual English/Spanish preferred
