We are looking to hire a qualified and compassionateLicensed Vocation Nurse (Floor) for their Primary Care Department.Â
Reporting to the Director of Operations, the LVN underthe direction of a physician, advanced practice provider, or Registered Nurse,delivers compassionate patient care and works collaboratively with thehealthcare team to promote health, prevent illness, and support positivepatient outcomes.Â
Schedule: Full-Time | 1.0 FTE
What You’ll Do Â
Key Responsibilities Â
- Demonstrate the health center’s mission by providing quality care and serving as an LVN resource to the care team, as applicableÂ
- Perform nursing duties within scope, including wound care, immunizations, and patient health educationÂ
- Complete medication reconciliation and administer prescribed medications and treatments per written or standing ordersÂ
- Maintain up-to-date emergency kits and oxygen tanksÂ
- Complete required forms in accordance with health plan and program standardsÂ
- Performs all duties in support of successful EHR and patient portal management Â
- Contact patients to schedule appointments as neededÂ
- Participate in care team and other assigned meetingsÂ
- Deliver services in full compliance with the health center’s standardsÂ
- Respond to and appropriately escalate provider and patient messages using SBAR and established health center policies and proceduresÂ
- Communicate through messages with external providers and agencies regarding patient care, issue resolution, and escalationÂ
- Respond to patient portal requests for forms, labs, and test results within scopeÂ
- Perform other related duties as assignedÂ
What We Are Looking For Â
License, Certification and Experience Â
- High school diploma or equivalentÂ
- Current, valid California LVN licenseÂ
- BLS certification requiredÂ
- 1+ year of LVN experience; outpatient clinic experience preferredÂ
