Key Responsibilities

  • Develop and update individualized care plans
  • Provide in-home physical therapy services per physician’s plan of care
  • Assess functional status, mobility, and home environment
  • Educate patients and families on therapy techniques and equipment
  • Maintain accurate documentation and communicate progress to physicians

Requirements

  • Licensed Physical Therapist in the State of Arizona
  • Degree from a CAPTE-accredited program; passed state-approved exam
  • Minimum 2 years’ experience (home health preferred)
  • Current CPR certification
  • Fingerprint Clearance Card (or immediate ability to apply)
  • Valid driver’s license, insurance, and reliable transportation
