Licensed Clinical Social Worker - LCSW (Hybrid)

Inland Empire, CA (San Bernardino)

This position is full-time and hybrid. 

We are seeking a full-time Licensed Clinical Social Worker who lives in the Inland Empire to provide behavioral health assessment, counseling, crisis intervention, and care coordination for patients with behavioral, social, and medical complexity. You will see patients where they are, in their homes, in the community, and in healthcare facilities, as part of an interdisciplinary team of physicians, nurse practitioners, nurses, care managers, and community health workers.

The focus of assistance is in enhancing the quality of patient management, maximizing patient satisfaction, promoting cost-effectiveness, and increasing the efficacy of addressing healthcare disparities. We specialize in engaging and caring for the highest utilizing Medicaid patients. We are looking for empathetic professionals that are enthusiastic about this population with a focus of care from that person's perspective.

Compensation and Benefits for Licensed Clinical Social Worker

  • Compensation: $70,000 to $105,000 annually, depending on experience

  • 3 weeks (2 weeks + 6-7 federal holidays) paid time away and 401K

Responsibilities for Licensed Clinical Social Worker

  • Complete behavioral health, psychosocial, functional, and social-needs assessments

  • Build and update individualized behavioral health and psychosocial care plans with patients and the care team

  • Provide counseling, supportive therapy, motivational interviewing, behavioral activation, and crisis intervention within LCSW scope

  • Assess and respond to safety concerns, including suicide risk, abuse, neglect, and domestic violence, and escalate to a higher level of care when needed

  • Coordinate with physicians, psychiatric providers, nurses, care managers, health plans, hospitals, skilled nursing facilities, and community organizations

  • Support transitions of care after emergency department visits, hospitalizations, and skilled nursing stays

  • Identify social determinants of health that get in the way of treatment and connect patients to the right resources

  • Educate patients, families, and caregivers on behavioral health needs, coping strategies, and community resources

  • Participate in interdisciplinary case conferences and care-team meetings

  • Complete clinical documentation accurately and on time, in line with HIPAA, mandatory-reporting, payer, and program requirements

Qualifications for Licensed Clinical Social Worker

  • Master's degree in Social Work from an accredited program (required)

  • Current, active, and unrestricted California LCSW license (required)

  • Working knowledge of behavioral health assessment, crisis intervention, and care coordination

  • Comfortable using electronic health records, care-management platforms, and telehealth tools

  • Experience with Medi-Cal, Medicare, dual-eligible, or other high-risk populations (preferred)

  • Experience in Enhanced Care Management, complex care management, integrated behavioral health, or population health (preferred)

  • Experience providing home- or community-based behavioral health services (preferred)

  • Experience with patients affected by housing instability, substance use, serious mental illness, or frequent hospital use (preferred)

  • Bilingual Spanish (preferred)
