Location: University Place, WA - Hybrid — home, office, and local travel throughout Pierce and King Counties

Job Schedule: Full-time or Part-time; Monday through Friday, within the hours of 8:00am – 4:30pm, plus participation in the on-call rotation as assigned

Pay: Based on education, licensure, and experience:

  • LICSW/LASW: $45.00 – $57.00 per hour
  • MSW/LSWAIC/LSWAA: $41.00 – $46.00 per hour
  • BSW/LSW or related bachelor’s degree: $35.00 – $40.00 per hour

 About the Role  

Family First At Home is looking for an experienced social worker who enjoys meeting clients where they are—often in their own homes—and helping families work through complicated healthcare and caregiving decisions.

This is a community-based role that combines psychosocial assessment, resource navigation, advocacy, counseling, and care coordination. Our Professional Care Managers support older adults and other clients living with chronic, acute, or terminal illnesses.

You will manage a caseload, develop care plans, connect families with appropriate services, and provide steady guidance during transitions or crises. The work is independent, but you will have a team available for consultation and collaboration.

Job Duties  

  • Meet with clients in their homes and complete initial and ongoing biopsychosocial assessments.
  • Assess clients’ health, safety, support systems, family dynamics, caregiver stress, financial concerns, housing needs, behavioral health, and access barriers.
  • Develop personalized care plans with clear, measurable goals and update those plans as the client’s needs change.
  • Coordinate care across clients’ homes, medical offices, hospitals, rehabilitation centers, senior living communities, and community providers.
  • Help clients and families understand their options and connect them with home health, hospice, home care, financial assistance, housing, legal aid, and other community resources.
  • Support safe transitions from hospitals, rehabilitation centers, or senior living communities back home or to another care setting.
  • Provide brief, solution-focused counseling and caregiver support within the scope of the role.
  • Advocate for clients during appointments, care conferences, crises, and changes in condition.
  • Stay in regular contact with clients and families and work closely with caregivers, healthcare providers, and the internal care team.
  • Document assessments, care plans, client contacts, progress, billable time, and other required information promptly and accurately.
  • Meet the position’s billable-hour and documentation expectations.
  • Transport clients to medical appointments or community resources when needed.
  • Participate in the Care Manager on-call rotation as assigned.

Qualifications 

  • BSW, MSW, or bachelor’s degree in psychology, health, or a closely related field.
  • At least 3 years of direct client service, clinical, or case-management experience.
  • Strong psychosocial assessment, documentation, communication, counseling, follow-through, and time-management skills.
  • Ability to work independently in clients’ homes and throughout the community.
  • Valid driver’s license, current auto insurance, and reliable transportation.

Preferred Experience

  • Current Washington LSWAIC, LSWAA, LASW, or LICSW credential.
  • 2+ years of experience in home health, hospice, home care, public health, aging services, community-based healthcare, or a similar setting.
  • Experience supporting older adults, people living with chronic or terminal illnesses, and family caregivers.
  • Experience with discharge planning, care transitions, advance care planning, placement support, or crisis response.
  • Certified Care Manager credential, such as CCM or CMC.

Travel and Mileage Reimbursement

Regular travel throughout Pierce and King Counties is part of this position. Business mileage incurred while traveling between work locations or visiting clients and referral partners will be reimbursed in accordance with the company’s mileage reimbursement policy.
