Summit County Public Health is searching for a Licensed Social Worker to work full time (35 hours per week) in our Senior & Adult Services program Monday through Friday. The Licensed Social Worker (LSW) supports our Senior and Adult Services program, which ensures health equity for older adults in Summit County through wellness, prevention, healthcare access, and safety initiatives. As an LSW, you will assess, plan, and implement strategies to enhance the health and independence of our older adult community members.

Key Responsibilities:

• Provide case management through the GateKeeper program to support safe, independent living for older adults.
• Assess the specific needs of older adults to ensure tailored, independent-living support services.
• Provide BRI Care consultations to support individuals living with dementia and their family caregivers.
• Conduct targeted outreach for caregivers, focusing on effective communication, self-care, and available resources.
• Facilitate educational initiatives on brain health, fall prevention, and dementia capability.
• Partner with initiatives such as Dementia Friends Ohio to build a more age-friendly Summit County.
• Collaborate with local community groups and partners to expand service reach and support.
• Administer program grants while ensuring accurate and timely documentation for all services.

Compensation & Total Rewards

This is a full-time position with a standard 35-hour work week.

• Compensation: $52,397.80 annually (equivalent to $28.79 per hour working 35 hours per week).

Qualifications:

• Bachelor's degree in Social Work or related field.
• Active LSW license through the State of Ohio Counselor, Social Worker & Marriage & Family Therapist Board.
• Minimum of 3 years related work experience
• Knowledge of resources and services in Summit County for individuals and families.
• Experience with multisystem, multi-need individuals and families, treatment planning, coordination of services, linking services, and record keeping.
• Valid Ohio driver's license, automobile availability and insurance coverage.
