Make a Difference—One Patient at a Time

Klarus Home Health is seeking a Licensed Occupational Therapist (OT) to join our growing home health team. This PRN/Part-time role offers one-on-one patient care, flexible scheduling, and the opportunity to work independently while fully supported by a collaborative clinical team. This position will cover the Northern Albuquerque (Bernalillo) region up to to Santa Fe, NM.

Requirements

 What You’ll Do

• Perform OT evaluations and ongoing assessments, including OASIS as required
• Develop, implement, and revise individualized plans of care
• Provide therapeutic interventions for ADLs, ROM, strength, coordination, and adaptive equipment
• Educate patients, families, and caregivers
• Maintain timely and accurate documentation
• Collaborate with physicians and interdisciplinary teams
• Supervise COTAs and aides as applicable
• Follow infection control and safety standards


What We’re Looking For

Required:
• Bachelor’s degree in occupational therapy
• Current New Mexico OT license
• Valid driver’s license, reliable transportation, and auto insurance
• Current CPR certification
 

Preferred:
• 1+ year OT experience
• Home health experience
• Homecare Homebase proficiency
• Strong communication skills


Physical & Work Environment Requirements

• Ability to lift, bend, push, pull, and stand/walk for extended periods
• Manual dexterity and hand-eye coordination
• Ability to work independently in patient homes
