Your Role
As a Prava Physical Therapist, you'll provide exceptional clinical care while becoming a trusted part of the MonteCedro community.
You'll have the opportunity to:
Complete comprehensive physical therapy evaluations
Develop individualized plans of care based on clinical findings and resident goals
Provide skilled treatment for balance, gait, strength, mobility, endurance, and functional performance
Help residents recover following illness, injury, hospitalization, or a change in functional status
Build treatment around meaningful, real-world goals
Provide resident and caregiver education
Collaborate with nursing, wellness professionals, care teams, families, and other clinicians
Identify changes in mobility and function that may benefit from skilled intervention
Support fall prevention, mobility, wellness, and independence initiatives
Incorporate residents' actual environments and daily routines into treatment
Communicate clearly with residents, families, and the interdisciplinary team
Document efficiently using Prava's modern technology and AI-enabled tools
Contribute to a clinical culture built around exceptional care and continuous learning

We're Looking for a PT Who:
Holds a current California Physical Therapist license
Has strong clinical judgment
Loves working with older adults
Thinks functionally rather than simply diagnostically
Builds genuine relationships with residents
Communicates exceptionally well
Is comfortable working independently
Collaborates naturally with interdisciplinary teams
Is curious and committed to continued clinical growth
Believes residents should be challenged appropriately
Understands that independence, dignity, and choice are fundamental to good care
Senior living experience is a plus—but it isn't everything.
