Experience:

3 years of post鈥恎raduate experience in a social work setting if Master's Degree is held; 1 year within military setting.

5 years of post鈥恎raduate experience in a social work setting if Bachelor's Degree is held; 1 year within military setting.

Duties & Responsibilities:

  • Provide non-clinical case management for eligible members with behavioral health and substance use disorders whereby care is coordinated and/or integrated across all elements of complex healthcare systems (e.g., the VA, subspecialty care, hospitals, local providers), patient's community (e.g., family, public and private community based services) and internal systems such as the Integrated Mental Health System (IMHS).
  • Provide care to eligible members, facilitated by registries, information technology, health information exchange, and other means to assure that eligible members get the indicated care when and where they need it in a culturally and linguistically appropriate manner.
  • Coordinate non-clinical resource referrals pertaining to housing, finance, food, shelters, utility assistance, mortgage relief, etc.
  • Immediately refer crisis situations with mentally unstable, suicidal, high risk populations
  • Attend events to engage, develop, and vet community partnerships and learn of new community resources available to assist eligible members.
  • Provide behavioral health briefings at Yellow Ribbon and similar events.
  • Ensure proper signing/handling of consent forms.
  • Prepare case management and critical cases; submit to COR on a monthly basis.

Must Haves:

  • Knowledge of federal and state legal requirements pertaining to mental health counseling and patient confidentiality.
  • Knowledge of HIPAA and Privacy Acts in civilian and military settings as well as the professional ethics surrounding the mental health career field.
  • Strong organizational & communication skills, both written and verbal
  • One year experience working with a Computerized Medical database system preferred.
  • One year experience working on a virtual team preferred.
  • Current certification or licensure in evidence based treatment for psychological trauma.
