Position Purpose:Â A proficient Memory Care Coordinator ensures that residents receive high-quality, individualized care in compliance with all regulations, ultimately enhancing the quality of life for individuals with memory impairments seamless, successful transition and warm welcome into our community.

Required Qualifications
- Experience: Minimum of 1–2 years in memory care, assisted living, or dementia-related services;
- Strong computer and internet skills, including Microsoft Office suite.
- Certifications: CPR certification is required; dementia care certifications such as CDP (Certified Dementia Practitioner) are a plus
- Core Skills: Strong communication, organizational, and leadership skills; empathy, patience, and a deep understanding of memory impairments; ability to multi-task and prioritize effectively
- Able to clearly present information, accurately communicate, provide necessary level of detail even under stressful or demanding conditions.
- Available to work a flexible schedule, which will include working days, evenings, weekends, and holidays.
- Strong organization skills, able to multi-task, and can manage time to meet frequently changing deadlines in a fast-paced environment.
- At least one year of experience in a nursing home community is preferred.
- Major Duties and Responsibilities
- Coordinating with Director, social workers, and family members to provide comprehensive care
- Monitoring patients’ health and behavior and making recommendations to Director as necessary
- Organizing and leading memory-enhancing activities, such as music therapy, art therapy, and reminiscence therapy
- Ensuring a safe and comfortable environment for patients
- Providing emotional support and counseling to patients and their families
- Maintaining detailed records of patients’ progress and care
- Implementing and maintaining compliance with all state and federal regulations related to memory care.

- Meet general health requirements according to facility policy, including medical and physical exams and checking immunity status to various infectious diseases.
- Ability to work beyond normal working hours and on weekends and holidays when necessary.
- Ability to assist in evacuation of residents during emergency situations.
- Ability to bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position.
- May be subject to falls, burns from equipment, and/or odors throughout the day; encounter reactions from dust, tobacco smoke, disinfectants, and other air contaminants.
- Subject to exposure to infectious waste, diseases and/or conditions which include AIDS, Coronavirus, Hepatitis B, and Tuberculosis.
- May be subject to hostile or emotional residents, family members, visitors or personnel.
