What You’ll Be Doing

The Care Coordinator is a central member of the on-site clinical team, helping ensure that patients with complex medical needs receive coordinated, responsive, and thoughtful care.

Working closely with our Care Coordination Manager, Advanced Practice Provider, physicians, and facility team, you’ll help organize each patient’s care, anticipate needs, and make sure nothing falls through the cracks. You’ll build relationships with patients and families, coordinate appointments and transitions, prepare the clinical team for efficient visits, and follow through on the details that can make the difference between a patient remaining stable in the facility and ending up back in the hospital.

This is an on-site, hourly role with a predictable Monday–Friday, 8:00 a.m.–4:00 p.m with significant flexibility to work remotely in the afternoons, with no evening or weekend requirements. It’s an ideal opportunity for an LPN or experienced Medical Assistant who enjoys patient relationships, thrives on organization and follow-through, and wants to play a meaningful role on a collaborative clinical team.

Key Responsibilities

  • Serve as a warm, responsive point of contact for patients and families, answering questions, coordinating needs, and ensuring every request is followed through to resolution.
  • Proactively connect with patients and families to understand clinical needs, provide updates, and schedule ongoing follow-up.
  • Coordinate with physicians, specialists, pharmacies, hospitals, and other healthcare partners to schedule appointments, obtain records, and keep care moving forward.
  • Help identify and prioritize patients who need medical attention each day, build the provider schedule, and prepare charts so clinicians can deliver efficient, high-quality care.
  • Partner closely with the on-site Advanced Practice Provider, facility nursing, therapy, social work, and other team members to keep care and communication seamless.
  • Monitor hospitalizations and emergency department visits and ensure the Fortis Care team has the information needed for timely follow-up.
  • Support transitions of care after hospitalization, including arranging follow-up, confirming medication reconciliation, and reinforcing discharge instructions.
  • Provide patients and families with appropriate educational materials and assist with communication related to specialty referrals and appointments.
  • Track open items and future follow-ups to ensure patient needs are addressed promptly and consistently.
  • Document all care coordination and care management activities accurately and in a timely manner.

What Makes Someone Successful in This Role

You don’t need to know everything on day one. What matters most is that you care deeply about patients, communicate exceptionally well, and take ownership of getting things done.

You’ll be a great fit if you:

  • Are warm, professional, and genuinely enjoy working with patients and families.
  • Communicate clearly and promptly by phone, in person, and in writing.
  • Take pride in being organized and following through on the details.
  • Can juggle multiple priorities without losing sight of what matters most.
  • Are comfortable working independently while staying closely connected to a clinical team.
  • Enjoy solving problems and finding practical ways to make care easier for patients, families, and providers.
  • Understand the basics of chronic disease management and are eager to continue learning.
  • Bring patience, empathy, and good judgment to interactions with older adults and their families.
  • Appreciate a high-service environment where responsiveness and professionalism matter.

Qualifications

  • Licensed Practical Nurse (LPN) or Medical Assistant (MA) required.
  • At least 1–2 years of experience in a clinical setting such as primary care, skilled nursing, long-term care, ambulatory care, chronic care management, or a hospital environment.
  • Familiarity with chronic medical conditions, preventive care, and patient education.
  • Experience working in an EHR or care management platform preferred.
  • Associate’s degree or higher in a clinical or health-related field preferred.
  • Prior care coordination, quality improvement, or population health experience is a plus.
  • Experience in a concierge, hospitality-oriented, or other high-service environment is also a plus.

Schedule & Benefits

We want this to be a role where you can do meaningful work and still have predictability outside of work.

  • Hourly, non-exempt position.
  • Monday–Friday, 8:00 a.m.–4:00 p.m. with flexibility to work remotely in the afternoons
  • No evenings, weekends, or call.
  • Flexibility to step away outside of core care coordination and team meeting times when needed.
  • Healthcare reimbursement account that can be used toward medical, dental, and vision expenses.
  • 401(k) plan after one year of employment.
  • 10 days of paid vacation plus 5 paid holidays.
  • Family-friendly work environment.

Why Fortis Care

At Fortis Care, we believe some of the most important work in healthcare happens outside the hospital.

Our physician-led team partners directly with skilled nursing and rehabilitation facilities to bring consistent, high-quality medical care to patients where they live and recover. Rather than simply responding when something goes wrong, we work proactively to identify problems early, coordinate care across specialists and settings, support patients and families, and reduce avoidable trips to the hospital.

Care Coordinators are essential to that mission. You’ll be much more than someone making phone calls or completing tasks. You’ll become a trusted resource for patients and families and an integral part of the clinical team, helping make sure the right care happens at the right time.

At Fortis Care, we value people who take ownership, communicate openly, and care deeply about doing right by patients and our partners. You’ll work alongside a growing, collaborative team and have the opportunity to help shape a different kind of post-acute care model as we continue to grow.
