Pay Rate: $23.00 - $25.00 per hour (Based on Experience)

What You’ll Do:

 As a Field Medical Assistant, you’ll play a vital role in driving exceptional patient care and ensuring seamless coordination across our healthcare team—providing oversight, guidance, and exceptional care to a dedicated caseload of patients. You’ll play a vital role in ensuring seamless communication, coordination, and support across providers, patients, and families.

What You’ll Do:

• Conduct engaging patient assessments and develop personalized care plans.

• Collaborate with healthcare providers to deliver smooth and efficient care.

• Manage referrals and follow up with patients to ensure timely care.

• Coordinate communication between patients, providers, and family members—keeping everyone aligned.

• Provide support exclusively in the field!

• Demonstrate professionalism and excellent phone etiquette in every interaction.

• Partner with multiple providers across the Access Healthcare Associates network.

• Monitor patient progress and adjust care plans to fit individual needs.

• Educate and empower patients and families with resources and guidance.

• Apply resource management skills to optimize care delivery.

• Conduct diagnostic evaluations and recommend tailored treatment options.

• Support team goals with additional duties as needed

• Perform other duties as assigned to support patient care and team goals.

Who You Are:

• You have a Medical Assistant Certificate (Required)
