Behavioral Health Clinician II Integrated Services

Position Title - Behavioral Health Clinician II - Integrated Services (Full-time)

Work Schedule - Monday - Friday 8:00 am - 4:30 pm EST

The Collaborative Care Behavioral Health Consultant functions as a core member of a Collaborative Care team that involves the patient’s primary care provider (PCP) and a psychiatric prescriber. The consultant is responsible for coordinating and supporting mental health care within the provider’s practice and for coordinating referrals to clinically indicated services outside the practice. Consultants support the treating physician in his/her role by coordinating treatment, providing proactive follow-up of treatment response, alerting the physician when the patient is not improving, supporting medication management, and facilitating communication with the psychiatric prescriber regarding treatment changes. They also offer brief counseling (using evidence-based techniques such as motivational interviewing, behavioral activation, and problem-solving treatment) and help facilitate changes in treatment if patients are not improving as expected.

Education & Training: Master’s degree in Counseling or Social Work, or Doctoral degree in Psychology. Licensed as LCSW-C or LCPC or Psy.D.

Work Orientation & Experience: Behavioral Health care experience required. Experience with screening for mental health and substance use disorders. Familiarity with brief, structured, intervention techniques. Experience working with patients that have co-occurring mental health, substance use, and physical health problems. Experience with evidence-based counseling techniques.

Skills & Abilities: Demonstrated ability to collaborate effectively in team setting; Ability to maintain effective and professional relationships with patient and other members of the care team; Strong communication skills; Ability to effectively engage patients in a therapeutic relationship, when appropriate; Ability to work with patients in person or by telephone; experience with brief assessment and treatment planning for common mental health and/or substance use disorders; Working knowledge of differential diagnosis of common mental health/substance use disorders; Working knowledge of evidence-based psychosocial treatments; Familiarity with brief, structured intervention techniques; Basic knowledge of psychopharmacology for common mental health disorders that is within the appropriate scope of practice. Computer skills required. Flexibility to work within changing environments in healthcare. Works well in fast paced environment.

Skills & Abilities: Demonstrate knowledge/skills of behavioral health clinical practice in a variety of settings: a.) Completing psychosocial assessments; b) provision of trauma-informed individual, group, and family therapy services; c.) developing and implementing discharge and aftercare plans; d) coordinating services for patient/family with community providers including knowledge of community resources, Medicare/Medicaid and commercial insurances; e.) knowledge of state and federal regulations as it pertains to scope of clinical practice in multiple settings; f.) effective critical thinking; g.) professional and effective written and verbal communication; h.) prioritize work assignments and manage time effectively to complete duties; and i.) work effectively and efficiently in a fast-paced environment.

Ability to a) communicate and collaborate effectively with both internal and external partners (i.e., behavioral health team, medical team, community stakeholders and patients/families); b) assess, adapt, and calmly respond to changing and/or crisis environment; c) make independent decisions consistent with current policies, procedures, health care and ethical standards; and d) be available to adapt hours to meet the needs of partners.

Compensation:

Pay Range: $35.08 - $52.64
