St. Charles Health System is a 13-bed acute rehabilitation unit dedicated to helping patients who are recovering from various injuries or illnesses. In partnership with Lifepoint Rehabilitation, this facility provides 24 hours of nursing care and intensive inpatient therapy to provide an outstanding experience and quality outcomes for all of the patients we serve.Â
How you'll contribute
Â
A Social Worker Coordinator who excels in this roleÂ
Â
Participate in pre-admission screening as requestedÂ
Conduct initial assessmentsÂ
Develop individualized treatment plansÂ
Provide age appropriate therapeutic interventionsÂ
Evaluate patient and family response to treatmentÂ
Document the course of patient care including progress made and continuing need for treatmentÂ
Establishes and maintains a therapeutic relationship with patients and familiesÂ
Provide patient and family education on tasks and resources that will assist with continued functional independenceÂ
Obtain pre-certification and re-certification of patient's insurance coverage as appropriate according to policy and procedureÂ
Participate in continuing education/professional development activitiesÂ
Communicate and demonstrate a professional image/attitude for patients, families, clients, coworkers and othersÂ
Contribute to client relations and marketing activitiesÂ
Maintain competency,  licenses/ certifications,  and memberships in applicable professional organizationsÂ

What we're looking for
Â
At a minimum, be a graduate of a CSWE approved master’s degree program and hold applicable and current license/certification to practiceÂ
ACSW certification is desirableÂ
Education/experience in working with an adult and geriatric population is necessary; education/experience serving people with physical disabilities is desirableÂ
