Job DetailsJob Location: Blountstown, FL 32424Position Type: Full TimeEducation Level: 2 Year DegreeTravel Percentage: Road WarriorJob Shift: NightJob Category: RN Job Summary As a Full-Time On-Call Registered Nurse, you’ll provide compassionate, responsive nursing care to patients and families during the weekend hours when they need support the most. As an important member of our On-Call Team, you’ll help patients and caregivers navigate changes in condition, manage symptoms, and feel supported throughout the end-of-life journey. This role is ideal for an experienced, compassionate, and confident RN who is comfortable working independently, responding to changing patient needs, and making sound clinical decisions in the field. You’ll provide hands-on nursing care, symptom management, education, and emotional support while collaborating with our interdisciplinary hospice team to ensure continuity of care for every patient and family we serve. Full-Time Weekend On-Call Schedule: Friday at 5:00 PM through Monday at 8:00 AM Service Area: Bay, Washington, Holmes, Jackson, Calhoun, and Gulf counties Supervisory Responsibilities Provides supervision and clinical direction to Licensed Practical Nurses (LPNs) and Hospice Aides as needed. Duties & Responsibilities Provides quality patient care and manages direct care for patients and families as part of the Interdisciplinary Team (IDT), incorporating appropriate nursing interventions and follow-up. Participates in providing 24-hour/day, 7-day/week patient care, including on-call rotation coverage as needed.* Provides compassionate palliative nursing care to patients and families using appropriate nursing interventions that incorporate physical, psychosocial, spiritual, and cultural needs consistent with the established patient/family plan of care developed by the IDT.* Revises and updates the patient/family plan of care in response to changing patient and family needs, collaborating with other members of the IDT to ensure services remain consistent with the patient and family’s goals of care.* Completes all clinical documentation accurately and in accordance with the State Nurse Practice Act, professional hospice nursing standards, licensing and regulatory requirements, and organizational policies and procedures, completing documentation at the point of care as appropriate.* Communicates effectively and compassionately with patients, families, physicians, vendors, pharmacies, facility staff, and other members of the care team to support seamless coordination of patient care.* Demonstrates strong clinical skills in case management, BBH policies and procedures, intravenous therapy, pain medication titration, symptom management, and patient/family relationships.* Demonstrates competence across a variety of hospice visit types and care situations, including on-call visits, death events, continuation of Continuous Home Care, admissions, discharges, transfers, triage revocations, and General Inpatient (GIP) care.* Provides and reinforces education to patients, families, and caregivers designed to increase their confidence, comfort, and independence in providing patient care and support.* Travels throughout the assigned service area and provides patient care in adjacent counties as situations warrant.* Performs other duties as requested.* Designates an essential function of the position. Demonstrates integrity, professionalism, discretion, and confidentiality in all interactions. Familiarity with Electronic Medical Record (EMR) systems and the ability to complete timely and accurate clinical documentation. Must possess a current Florida Driver’s License, valid auto insurance, and reliable transportation for travel throughout the assigned service area. Education & Experience Current and valid Florida Registered Nurse (RN) license required. Bachelor’s Degree in Nursing (BSN) required. Minimum of three years of nursing experience, preferably in hospice or home health. Hospice or palliative care experience required. Certified Hospice and Palliative Nurse (CHPN) certification preferred. Physical Requirements This position requires the ability to safely perform the physical and travel-related responsibilities associated with providing hospice care in patient homes, facilities, and other care settings. Ability to sit for prolonged periods while completing documentation and working on a computer. Ability to lift up to 15 pounds as needed. Ability to safely access and navigate patient homes, healthcare facilities, organizational locations, and other care environments. Ability to travel throughout the assigned service area to provide patient care as needed.
