At Mattapan Health & Rehabilitation Center, our Registered Dietitians play an important role in supporting the health, recovery, comfort, and quality of life of the residents we serve.

We are seeking a compassionate and clinically strong Registered Dietitian to join our interdisciplinary team within our 85-bed skilled nursing and rehabilitation community.

This role provides an opportunity to make a direct impact through individualized nutritional care planning, clinical collaboration, resident education, and ongoing support for both our Short-Term Rehabilitation and Long-Term Care populations.

Position Overview

As a Registered Dietitian, you will:

  • Conduct nutrition assessments for short-term rehabilitation and long-term care residents
  • Develop and monitor individualized nutrition care plans
  • Provide nutrition counseling and education to residents and families
  • Monitor residents' nutritional status, weight trends, intake, and overall wellness
  • Collaborate with Nursing, Dining Services, Therapy, medical providers, and other members of the interdisciplinary team
  • Help ensure regulatory compliance and evidence-based nutritional practices
  • Maintain accurate nutrition documentation within the electronic medical record
  • Support overall resident wellness, recovery, and quality outcomes

This role serves as an important clinical resource within the community and plays a key part in providing safe, effective, and compassionate resident care.

Responsibilities

Registered Dietitian (RD / CDN):

 

Clinical Nutrition & Resident Care

  • Complete nutrition assessments and reassessments for residents
  • Develop, implement, and monitor individualized nutrition care plans
  • Provide nutrition education and counseling to residents and families
  • Monitor residents’ nutritional status, weight trends, intake, and overall wellness
  • Maintain accurate nutrition documentation within the electronic medical record system

Collaboration & Team Partnership

  • Participate in interdisciplinary care plan meetings
  • Partner with Dining Services to support appropriate meal planning and service
  • Serve as a nutrition resource for facility leadership and clinical teams
  • Support and mentor Diet Technicians and dietetic interns, as applicable

Quality, Compliance & Education

  • Conduct nutrition-related audits and performance improvement initiatives
  • Maintain compliance with CMS, DOH, and nutritional service regulations
  • Assist with staff education and in-service training programs
  • Support survey readiness and best practice initiatives

Qualifications

Registered Dietitian (RD / CDN):

  • Bachelor’s degree in Nutrition, Dietetics, or related field
  • Registered Dietitian credential required
  • Active registration with the Commission on Dietetic Registration (CDR)
  • Certified Dietitian-Nutritionist (CDN) or state licensure, as applicable
  • Long-term care, rehabilitation, or geriatric experience preferred
  • Strong communication and interdisciplinary collaboration skills
  • Experience with nutritional software and electronic medical records preferred
