DUTIES AND RESPONSIBILITIES: Develop person-centered service plans and conduct assessments as required by the program. Conduct monthly face-to-face visits with participants. Assist clients with accessing community resources. Monitor participant progress with waiver services, including health, safety, and welfare in monthly contact notes. Assist with building a network of supports. Assure participant choice and access to community resources. Return phone calls and emails promptly. Complete and submit documentation by required deadlines. Perform other related work as required and assigned. QualificationsBachelor’s degree in Social Work, Human Services, or a related field preferred. Candidates may also qualify with one of the following: Bachelor’s degree in any field with at least one year of human services-related experience. Associate degree in a behavioral science, social science, or related field with at least two years of human services-related experience. Three years of human services-related experience. Relevant fields may include Social Work, Psychology, Rehabilitation, Nursing, Counseling, Education, Gerontology, Human Services, or Sociology. Relevant experience may include case management or direct work with individuals with intellectual/developmental disabilities, mental health conditions, or other vulnerable populations, including roles such as CNA, CMA, home health aide, personal care assistant, or similar. Unpaid experience such as internships, volunteering, and caring for a family member may also be considered. Additional Requirements: Valid driver’s license and auto insurance. Strong organizational skills. Ability to multi-task. Excellent communication skills. Self-motivated. Ability to conduct and facilitate meetings. Ability to collaborate with other team members. Ability to advocate for the needs of participants. JOB REQUIREMENTS / EXPECTATIONS: This job consists of sitting to complete necessary paperwork on a computer. Must be able to operate a motor vehicle and have reliable transportation with valid insurance. Must provide verbal and written communication with team members. Job setting is at the corporate office and/or home office setting. Documentation is time sensitive and must follow waiver regulations. Work schedule is Monday-Friday 8:00 AM-5:00 PM.
