Provides one-on-one job coaching and support to individuals with disabilities or other barriers to employment. Assists individuals with application and job preparation. Conducts skills assessments to determine fit for job duties.  Develops and implements individualized program plans to support job readiness and success. Provides ongoing follow-up and support to ensure job satisfaction and retention.   Provides on-site vocational direction and support to persons served. Builds and maintains strong relationships with employers, community organizations, and government agencies to secure and maintain job placements. Schedules appointments and ensures transportation for individuals served. Prepares and maintains records within program guidelines. Works within facilities employing persons served to build skills and ensure success. Ensures work duties are satisfied for persons served.   Coordinates assigned worksite operations and staffing. Makes reasonable efforts to maintain the required staffing level per shift and promptly reports vacancies, call-offs, or coverage barriers to the designated worksite supervisor per program guidelines. Prepares worksite schedules and coordinates scheduling and time-off coverage with other job coaches to support appropriate coverage each workday. Assists with recruitment by identifying staffing needs and coordinating with the worksite and partner agencies. Serves as a primary day-to-day liaison among Prestera, worksite representatives, job coaches, and partner agencies while maintaining professional and collaborative working relationships. Coordinates daily workflow and coverage but does not have authority to hire, terminate, discipline, approve leave, change compensation, or make contractual commitments unless specifically authorized in writing. QualificationsPrimary Attributes Professional & Technical Knowledge: Must possess a high school diploma or General Educational Development (GED) certificate. Possesses specific knowledge of the mental health, substance use disorder, and disability populations and of supported employment processes and practices, typically learned on the job or through a series of training sessions. Continuing Education: Maintains current knowledge of standards of care and practices, typically acquired through continuing education. Licenses & Certifications: A valid state-issued driver’s license. Technical Skills: Prepares basic correspondence, schedules, and reports in Microsoft Word. Uses Microsoft Excel to create tables, schedules, and simple displays of information. Creates basic presentations in Microsoft PowerPoint. Demonstrates necessary proficiency with all electronic clinical systems, including EHR and scheduling systems, in use at the Company. General Development: Functions effectively with general direction from supervisors and independently coordinates routine worksite activities. Selects from pre-established alternatives to resolve situations encountered on the job, identifies when issues require escalation, and communicates timely and accurate information through the established chain of command. Able to effectively communicate opinions drawn from conclusions using inference and logic. Exercises tact and diplomacy in resolving mild conflicts or disagreements and in working with employees, persons served, employers, and partner agencies. Work Environment: Work is performed independently in the field, primarily in a work setting. Physical demands include frequent walking, standing, and possibly climbing stairs in facilities without elevators. The position may occasionally involve lifting or carrying materials.
