JOB SUMMARY

As an important team member of Saint Francis, the MyCare Connect Coordinator utilizes critical thinking; the position evaluates care journey needs, aligns patients with appropriate care, and facilitates timely and coordinated patient access. Additionally, the position triages patient friction points, and serves as a liaison between patients, providers, and interdisciplinary teams to ensure coordinated, efficient, and high-quality care delivery.

Type: Full Time (80 hours per 2 week pay period, with benefits) 

Typical hours for this position: Monday-Friday 8:00am-5:00pm

Education: 

- High School diploma or GED- required
- Associate degree- preferred 

Experience: 
- Clinical medical office experience including scheduling- preferred
