Job Overview

Elizabeth Care Center is seeking a compassionate and experienced Physical Therapist (PT) to join our rehabilitation team. In this role, you will help residents improve mobility, restore function, manage pain, and achieve their highest level of independence through individualized, resident-centered care.

The Physical Therapist will evaluate residents, develop and implement treatment plans, collaborate with the interdisciplinary team, and provide clinical supervision to two Physical Therapy Assistants (PTAs).

Key Responsibilities

  • Perform physical therapy evaluations, reassessments, and individualized treatment planning. 
  • Provide skilled treatments using appropriate therapeutic techniques and modalities. 
  • Monitor resident progress and modify treatment plans as clinically appropriate. 
  • Provide clinical direction and supervision to two PTAs and other assigned rehabilitation staff. 
  • Collaborate with physicians, nursing, therapy, and other disciplines to coordinate resident care. 
  • Educate residents and families regarding treatment plans, progress, safety, and continued care. 
  • Maintain accurate, timely, and compliant clinical documentation. 
  • Communicate significant changes in resident condition to appropriate providers and team members. 
  • Maintain therapy equipment and a clean, safe treatment environment. 
  • Participate in staff education, infection prevention, safety, and quality improvement activities. 
  • Support department operations, including supplies, orientation, and staff development.

Requirements

 

Requirements

  • Master's or Doctorate degree in Physical Therapy. 
  • Current and active Physical Therapist license in West Virginia. 
  • Strong clinical assessment, treatment planning, and therapeutic skills. 
  • Ability to supervise and provide clinical direction to PTAs. 
  • Strong communication, organization, and interpersonal skills. 
  • Ability to work independently and collaboratively. 
  • Ability to maintain accurate and timely resident records. 
  • Knowledge of physical therapy techniques, modalities, and standards of practice. 

Physical Requirements: Ability to perform considerable physical activity, including lifting, pushing, pulling, or moving 50 pounds or more; stand and/or walk for more than four hours per day; and regularly bend or stoop as required to provide resident care.
