Description

O'Hare Outreach Worker will provide formal outreach and other clinical services to O'Hare Outreach Clients.


Essential Functions:

  • Provide coordinated outreach services to prepare potential clients to accept services and formal outreach services to triage and register clients.
  • Provide initial medical/psychological and additional assessments to registered clients leading to the definition of treatment issues, and continued care/aftercare planning to assigned patients and document same in patient charts.
  • Maintain assigned patient charts in manner which meets Haymarket Center's Quality Assurance standards and is clear its documentation of patient's progress in treatment.
  • Provide case management, referral, and referent contact services to assigned patients and document same in the patient's chart.
  • Monitor physical and emotional health status of patients and facilitate appropriate referral when assessed to need medical, psychiatric, dental evaluation, and care.
  • Provide crisis intervention as required and follow-up clinical assessment with patient(s) involved to determine need to appropriate clinical course of actions.
  • Attend patient staffing/interventions and document outcomes in patient chart.
  • Meet weekly with Supervisor or designee for clinical supervision.
  • Seek further supervision when appropriate and necessary.

Requirements

  • High School Diploma or Equivalent
  • Knowledge of and ability to perform all "Core Functions".
  • Must have a Valid Illinois Driver's License, in addition to meeting the agency requirements to become an approved driver.
