SUMMARYÂ
The Licensed Practical Nurse (LPN) I provides comprehensive skilled care as ordered by the attending physician by following the order sets created by the RNCM. The LPN I educates patients and their family members and ensures the safety of the patient. The LPN I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The LPN I is responsible for all practices and duties within the scope of practice as outlined by the state.Â
KEY RESPONSIBILITIESÂ
Provides skilled nursing care in a patient’s home as ordered by the attending physicianÂ
Performs care by following order sets and goals made by RNÂ
Observes and monitors patient conditionsÂ
Notifies RNCM of any changes in patient conditionÂ
Administers medication as prescribed by the physicianÂ
Counsels patient and family on the disease/injury processes and how to manageÂ
Incorporates patient and family in development of plan of careÂ
Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisionsÂ
Listens to patient and family membersÂ
Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelinesÂ
Documents all aspects of treatment, assessments, and patient educationÂ
Maintains active LPN license in state of employmentÂ
Communicates with scheduler any changes outside the normalÂ
Participates in all on-call requirements and case conferencesÂ
Attends in-service trainings and mandatory agency meetingsÂ
Maintains a professional appearance as a representative of the companyÂ
Understands and practices agency policies and proceduresÂ
Drives to patient’s primary location per scheduled visit.Â
Daily attendance at assigned visit locations.Â
Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines. Completes and submits all required documentation within specified company requirements.Â
Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse.Â
Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.Â
Other duties as assigned.Â
QUALIFICATIONSÂ
Minimum of one year of experience as a LPN in an acute care setting is required; two years is preferred. Home care experience is desired.Â
Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers.Â
Ability to remain calm, have patienceand be accommodating. Compassionate and caring while working with patients.Â
Knowledgeable on nursing best practices.Â
Ability to make appropriate nursing judgments.Â
Ability to identify a situation and handle it with the best possible solution.Â

Valid driver’s license and auto insurance with your name as a listed driver.Â
EDUCATION AND CREDENTIALSÂ
