Job DetailsJob Location: Colorado Springs Pikes Peak Location - Colorado Springs, CO 80909Position Type: Full TimeSalary Range: $19.00 - $21.85 HourlyPOSITION SUMMARY This is an administrative position that supports the day-to-day operations of the Interdisciplinary Team (IDT) by tracking and documenting multiple functions of the team. The IDT consists of various interdisciplinary team members who provide care to our participants. IDT Meeting Support Maintains the administrative materials needed for IDT meetings, such as sign-in sheets, agendas, and tracking information, and ensures they are prepared prior to IDT meetings. Attends and participates in IDT meetings. Manages the IDT meeting agenda, including receiving agenda points from team members via a variety of venues. Takes thorough and complete minutes for IDT meetings, as well as pre-enrollment IDT meetings. Assessment and Care Plan Coordination Oversees monthly assessment lists and assessment delays. Coordinates supplementary meetings required by the IDT, including taking minutes and documentation in the electronic health record (EHR). Tracks participant statuses and incidents, such as hospitalizations, emergency department visits, subacute rehab stays, scheduled surgeries/procedures, and falls; updates the huddle accordingly. Coordinates the interdisciplinary care plan schedule, including opening and closing care plans, and updates care plans with attendance to ensure regulatory compliance. Completes care plan communication with participants, caregivers, and community partners. Administrative and Documentation Support Provides administrative functions for the IDT, including maintaining email distribution lists. Tracks information and enters documentation in the EHR to ensure regulatory compliance related to IDT responsibilities of care plans, timely assessments, and service determination requests. Ensures IDT technician coverage for all IDT meetings. Performs other duties as requested/assigned. QualificationsEducation & Experience High School diploma required. Experience working in a healthcare setting preferred. Experience working in an electronic health record (EHR) preferred. Experience with Excel spreadsheets preferred.  Knowledge, Skills, and Abilities Excellent written and verbal communication skills required, including communicating with participants and caregivers over the phone.
