What you'll do

The Nurse Manager for Case Management and the Logistics Center is responsible for the recruiting, scheduling, on-boarding, training, coordination, support, and evaluation of the Unit Clinical Care Coordinators, Logistics Center Clinical Care Coordinators (Logistics Center C3s) and supporting positions. The Manager assumes operational responsibility for patient progression, continuous process improvement, and the management of human, fiscal and material resources for A.O. Fox Memorial Hospital, Cobleskill Regional Hospital, O’Connor Hospital and Little Falls Hospital. The Manager will ensure that operational activities are consistent with and in support of the organization’s strategic plan. Maintains current knowledge of basic health care economics, trends and reimbursements methodologies and applies knowledge to daily practice.

 

The manager plans for and secures financial and human resources for the department. Oversees, supports and implements the goals of Care Coordination and the Logistics Center. Maintains oversight responsibility for patient flow and through-put of all acute patients. Assists the Manager of Risk Management and Quality Improvement with process improvements, screening investigations and problem solving as needed. Responsible for educating staff on the implementation of clinical protocols. Participates in HMO contracts issues as needed. Manages quality improvement activities and oversees the competencies within the department.

  • Develops and monitors operational procedures and performance standards which ensure safe, effective and efficient daily operations.
  • Develops and monitors departmental policies and protocols which meet the needs for provision of the Logistics Center.
  • Develops strategies to ensure effective and efficient departmental performance and service delivery.
  • Monitors and reports departmental performance related to established objectives and goals.
  • Is responsible for ensuring timely patient placement (admissions, discharges, transfers) across the network.
  • Serves as a frontier leader for Operational Rounding, and facilitates Performance Improvement and A3 efforts for Case Management and the Logistics Center.
  • Audits of transfer cases (via Intake Logistics and Amtelco line) to ensure staff receive real-time feedback on Intake cases and to address any issues raised by referral sources, admitting providers, or other health system representatives.
  • Fosters a commitment to continuous quality improvement by supporting a coordinated system for evaluating a patient care outcomes, quality measures, milestone performance, and Key Performance Indicators.
  • Provides 24/7 managerial support to all Clinical Care Coordinators and Clinical Care Coordinators assistants (C3As).
  • Acts as a resource for escalation of issues experienced by the Clinical Care Coordinators. 
  • Develops staffing and scheduling to meet demands for operational services.
  • Provides direction to ensure appropriate use of staffing resources.
  • Develops and implements plans for staff development, orientation and professional growth.
  • Completes annual performance appraisals for all staff within established time-frames.
  • Manages and participates in staff recruitment, interview process and selection.
  • Assists C3s as needed with transfer center issues as they arise (ex. EMTALA, provider issues appropriateness of admissions, etc.)
  • Provides timely and effective communications with department staff to maintain awareness of important corporate, departmental and departmental information.
  • Schedules and directs staff meetings on a regular basis.
  • Makes use of staff feedback and insight to improve services.
  • Participates in multidisciplinary committee work groups and teams to support and facilitate initiatives and programs for service improvement.
  • Develops and implements annual operational budget and capital budgets.
  • Monitors and reports budgetary variances and emergency capital needs.
  • Reallocates, prioritizes and, when indicated, recommends modifications to established budgetary plans.
  • Develops and implements expense control measures to ensure financial objectives.
  • Works to develop and maintain revenue generating opportunities.
  • Audits DRGs and TLOS entry and appropriate status of patients admitted through the Logistics Center.
  • Takes action on identified areas for performance improvement by implementing plans and  evaluating effectiveness of improvement measures.
  • Tracks and reports performance improvement activities as appropriate and/or required by institutional guidelines.
  • Make recommendations for improvement to Director Case Management.
  • Ensures compliance with all regulatory agency requirements.
  • Ensures compliance with departmental and institutional policies.
  • Collaborates with the Director of Care Coordination on the development, review, and revision of policies and procedures consistent with the Institutional practice and Care Coordination.
  • Pro-actively identifies safety-related issues, addresses identified issues and institutes procedures which minimize risks to patients and staff.
  • Reports and acts to correct compliance and risk related issues to Director of Case Management.
  • Develops working relationships with partners throughout the Bassett Network to effect positive results.
  • Works collaboratively with unit staff (charge nurse, staff nurse) unit manager, and support team (UM, MSW, SW, etc) to progress each patient’s care toward discharge.
  • Supports regional capacity management activities and assures reports on the changes in affiliate capacity are communicated to the organization by Logistic Center C3s on a consistent basis.
  • Regularly assesses opportunities to enhance system(s) performance.
  • Develops action plans and completes them to improve patient and employee satisfaction.
  • Connects with counter parts at affiliate sites to actualize potential for adoption of best practices and standardization.
  • As appropriate, takes action to create a better experience for patients traversing the continuum of care.
  • Measures deployment of resources and develops plans to optimize ROI.
  • Serves as a consultant, coordinator and educator for the care coordination team related to issues of appropriate placement, patient progression, pre-certification and payer requirements, government regulations and appropriate statusing.
  • Performs other duties as requested and observed by Director of Case Management.

What we need from you

Education:

  • 4 Year / Bachelor's Degree in Nursing, required or will complete Bachelor’s Degree in Nursing within one year
    Or Social Worker

Experience:

  • Minimum 1 to 3 years of progressive, responsible healthcare supervisory experience; strong communication and interpersonal skills with ability to work effectively and collaboratively with nursing colleagues, physicians, administrators and the entire healthcare team, required

Licensure/Certifications:

  • Current NYS Registered Nurse License, required
  • PR and/or Screen Certification within 6 months of hire, required
  • Case Management Certification, preferred

Physical Requirements:

  • Sedentary Work - Exerting up to 10 lbs of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. We are also offering a sign-on bonus for this position! The bonus amounts are based on the position type and are listed below. Additional details and eligibility requirements will be discussed during the interview process. Bonus Amount:

  • RN (excluding management) sign-on bonuses are awarded in the amount of $20,000 for full time
  • LPN sign on bonuses are awarded in the amount of $10,000 for full time

Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. USD $96,960.00/Yr.
