This is a high-impact role for someone who thrives on building connections, developing referral partnerships, and making a difference in the lives of patients and families. As a Community Care Liaison, you will be the face of our hospice services in the community. You’ll build and strengthen relationships with physicians, hospitals, and other referral sources to increase awareness, trust, and utilization of our services. Your success will be measured by your ability to grow census through strong partnerships, consistent outreach, and a deep understanding of hospice care. You’ll also serve as a trusted resource—educating providers, patients, and families on the value of hospice and guiding them toward compassionate, high-quality care. Serve as a dedicated advocate for hospice by educating the community and sharing the value of comfort-focused, patient-centered care. Achieve and exceed census and admission targets by positioning our hospice as a preferred provider within the community. Engage regularly with healthcare providers and referral sources to create strong, collaborative partnerships focused on patient well-being. Lead hospice discussions with patients and families, clearly communicating services and guiding the consent process with professionalism and empathy. Education:                                Bachelor’s degree or equivalent  Experience: 1-2 years healthcare specific outside sales experience. 1-2 years of Hospice or Home Health sales Preferred.  Travel: Ability to perform regular travel within assigned geography and extended geography for training and meetings. Transportation: Must have reliable transportation. Valid and current driver’s license and current auto insurance required.
