JOB SUMMARY
Provides coordinated care in the home to clients and assists with creating patient centered action plans to promote effective education, self-management support, and adherence to the PCP provider's plan of care facilitating access and timely healthcare delivery.

Work Schedule: This is a PRN position, hours of 8-5pm (Monday-Friday, no weekends). 20 visits per week with flexible hours.

EXPERIENCE AND EDUCATION

Required

  • Education
    Master's Degree in social work 
     
  • Experience
    1 year of experience in a health care setting preferable with homebound patients. 
     
  • Licenses and Certifications
    Current license is required by the Texas State Board of Social Work Examiners. 
    (DRIVERS) DRIVERS LICENSE TX Driver's License and proof of driver's insurance 
     


JOB DUTIES

  • Ensures the delivery of quality social service for patients by identifying patient needs and developing social service programs to meet those needs.
  • Participates in development and update of Plan of Treatment.
  • Evaluates patient and home environment to determine social and psychological factors related to patient's health problems.
  • Prepares and submits to agency psychosocial histories, clinical and progress notes. Observes and reports on patient/family reaction to treatment.
  • Counsels' patient and/or family in understanding, accepting, and adjusting to patient's health problems and emotional care.
  • Identifies, informs, and refers patient/family to available community resources.
  • Coordinates discharge planning.
  • Interprets to the community the role of social work in the Agency's programs.
  • Participates in planning, development, and delivery of staff in-service education programs.
  • Participates in Continuing Quality Improvement Program. Monitors adherence to policies, procedures, and protocols.
  • Performs other related duties as assigned or requested.
