Position Summary

Southern Hearts Home Care of Georgia is seeking an experienced, dependable, and highly organized Registered Nurse (RN) to join our office team.

This position is responsible for providing clinical oversight of our field nursing and caregiving staff, including RNs, LPNs, CNAs, and other direct-care staff, while helping ensure that services are delivered safely, professionally, and in accordance with client plans of care, company policies, and applicable regulatory requirements.

The ideal candidate will have strong clinical judgment, leadership skills, excellent documentation practices, and experience in home care, home health, private duty nursing, or a related healthcare setting.

Key Responsibilities

  • Provide clinical oversight and support to RNs, LPNs, CNAs, and other field staff.
  • Review nursing documentation, clinical notes, assessments, care plans, and other required records for completeness and accuracy.
  • Complete, review, and sign nursing and clinical documents as permitted and required within the RN’s scope of practice.
  • Assist with initial assessments, reassessments, supervisory visits, and other nursing visits as needed.
  • Communicate with clients, families, physicians, field staff, office personnel, case managers, and other members of the care team.
  • Assist with developing, reviewing, and updating plans of care and ensuring field staff understand client-specific care requirements.
  • Provide clinical guidance and education to nursing and caregiving staff.
  • Help ensure field staff maintain required credentials, competencies, and clinical documentation.
  • Follow up on changes in client condition, incidents, hospitalizations, complaints, and other clinical concerns.
  • Assist with quality assurance, chart reviews, clinical audits, and preparation for regulatory surveys or inspections.
  • Complete clerical and administrative nursing duties, including reviewing records, preparing documentation, maintaining clinical files, and responding to clinical correspondence.
  • Work closely with staffing, human resources, administration, and other departments regarding clinical and patient-care matters.
  • Participate in orientation, training, competency evaluations, and ongoing education of clinical staff as assigned.
  • Maintain confidentiality and comply with HIPAA requirements and company policies.
  • Perform other nursing, clinical, and administrative duties related to home care operations as assigned.

Qualifications

  • Current, active Registered Nurse (RN) license in the State of Georgia in good standing.
  • Nursing experience required; home care, home health, private duty nursing, or supervisory experience preferred.
  • Strong knowledge of nursing documentation and clinical standards.
  • Ability to supervise and communicate effectively with RNs, LPNs, CNAs, caregivers, clients, and families.
  • Strong organizational, leadership, and problem-solving skills.
  • Excellent written and verbal communication skills.
  • Comfortable performing both clinical and office/clerical responsibilities.
  • Proficient with electronic medical records and basic computer systems.
  • Ability to manage multiple priorities and meet documentation deadlines.
  • Must maintain all credentials and requirements applicable to the position.
