Care Coordinator – Murphy Center 

  • Salary Range: $24.04 - $33.66 per hour, based on relevant years of experience
  • Location: Fort Collins (Murphy Center & Longview) 
  • Status: Full time, 40 hours per week
  • Hours: Monday – Friday 8:00am – 5:00pm 


Employees working at locations operating 24/7 earn a differential.  Shift differentials are also available for evenings, nights, weekends and holidays.

Role Overview:

This position is within our community care coordination program, specifically located as a hybrid position in the community at a day shelter for unhoused services in Fort Collins and at Longview Acute Care facility. This role provides care coordination services such as: assisting clients in connecting to resources, case management activities, assistance in navigating systems of care, and connection to services that support client's individualized behavioral health and housing goals. This role includes assisting clients with care coordination needs in a medical and day center setting. The Care Coordinator will engage in treatment planning collaboratively with the client and multidisciplinary team to ensure all needs are being appropriately addressed and met. This role requires collaboration with and across multi-disciplinary and multi-agency teams to serve clients across Larimer County. The care coordinator will have knowledge of diversity and inclusion with the ability to engage, communicate, interact, and work effectively and cooperatively with people of different backgrounds, identities, and culturally diverse communities.


Essential Duties: 

  • Provide care coordination services on behalf of and in collaboration with clients.
  • Support clients navigating health systems and increase continuity of care.
  • Participate as part of multidisciplinary teams and assist where needed within the scope of the position.
  • Engage with various stakeholders and community partners to coordinate care including but not limited to emergency departments, health services, primary care settings, acute care systems, crisis response teams, and behavioral health teams.
  • Collaborate with SummitStone staff and the Colorado Opioid Synergy Larimer and Weld (CO-SLAW) network.
  • Outreach clients on a referral and/or cold outreach basis.



Minimum Qualifications:

  • High school diploma required.
  • At least 1-year relevant experience with care coordination or case management in the Mental Health Field, Substance Use Disorder Field, or program of application.
  • Experience working with people who are experiencing homelessness.
  • Experience with mild to severe mental illness and substance use disorders.
  • Experience navigating systems of care.
  • A desire to work with historically marginalized communities.



Licenses and Certifications:

  • Valid Colorado driver’s license or the ability to obtain one within 30 days of hire required; must maintain a clean driving record. This role requires occasional travel to client homes and community partner sites.  


Preferred Qualifications:

  • Bachelors (BA/BS) Degree in psychology, social work or related field from accredited College or University. 
  • CAS, CAT, CAC III or LAC preferred.
  • Bilingual/bi-cultural preferred.
