Minimum Qualifications: 

  • To be trained as a lifestyle coach or leader within 3 months of hire, if leading any applicable classes.


Job Summary: 
Work with the Chronic Diseases Prevention and Self-Management Program team to enroll or lead program classes.

Job Duties:
Program Recruitment (50%25): 
•    Recruit program participants, adhering to each program's eligibility criteria. 
•    Participate in recruitment events, pitching the program information to potential participants. 
•    Participate in investigator meetings and workshops. 
•    Adheres to internal controls and reporting structure.

Lifestyle coach and leader (50%25): 
•    Conduct Chronic Diseases Prevention and Self-Management Program classes, as assigned.
•    Collect and report point-of-care blood work, biometrics, and surveys from program participants.
•    Record and report participant data and outcomes.

MARGINAL OR PERIODIC JOB FUNCTIONS: 
•    Performs related duties as required.


Preferred Qualifications: 
•    Earning a degree in public health, healthcare administration or related discipline
•    Bilingual (English - Spanish)
•    Experience in implementing lifestyle modification programs
•    Experience working in academic health centers
•    Experience in working with participants of all ages

 

Salary Range:

Commensurate with experience
