About the Role

Impact Life is seeking an engaging and compassionate Outreach Specialist to support our Sussex County community outreach initiative.

The Outreach Specialist is a frontline, community-based position focused on connecting individuals with treatment, recovery, healthcare, and supportive services. This role provides community outreach, harm reduction education and supplies, SBIRT screening, overdose prevention education, care navigation, and warm handoffs to treatment and recovery services throughout Sussex County.

The position is designed to reach individuals who may experience significant barriers to traditional healthcare and treatment, including people who are uninsured or underinsured, experiencing homelessness or housing instability, justice-involved, living in rural communities, or otherwise disconnected from services.

Key Responsibilities
  • Conduct community-based outreach and engagement throughout Sussex County.
  • Build trusting relationships using respectful, low-barrier, trauma-informed approaches.
  • Provide harm reduction education and distribute approved harm reduction and overdose prevention supplies.
  • Provide overdose prevention education, including information about naloxone and other risk-reduction strategies.
  • Conduct SBIRT screenings in accordance with training, program procedures, and the scope of the position.
  • Provide care navigation and help participants identify appropriate treatment, recovery, healthcare, and supportive services.
  • Provide warm handoffs to Impact Life outpatient treatment, recovery services, and community providers.
  • Support timely connections to rapid assessments and clinical services.
  • Help participants access Medication for Opioid Use Disorder (MOUD), outpatient treatment, and ongoing recovery supports when appropriate.
  • Connect participants with recovery housing, peer support, workforce development, primary care, behavioral health services, and other community resources.
  • Follow up on referrals and help address barriers to engagement and continuity of care.
  • Develop collaborative relationships with treatment providers, healthcare organizations, recovery organizations, community agencies, and referral partners.
  • Represent Impact Life at outreach activities, partner locations, resource events, and other community engagement opportunities.
  • Maintain accurate documentation of outreach contacts, screenings, referrals, warm handoffs, and participant engagement.
  • Collect and enter required program data and support achievement of grant outcomes and performance measures.
Required Qualifications
  • High school diploma or equivalent.
  • Knowledge of, or ability to learn, harm reduction, overdose prevention, SBIRT, substance use disorder, behavioral health, and community resource navigation practices.
  • Strong interpersonal and communication skills with the ability to respectfully engage individuals in community settings.
  • Ability to maintain confidentiality and appropriately handle sensitive participant information.
  • Strong organizational, documentation, and follow-through skills.
  • Ability to work independently in community settings while collaborating with clinical, recovery support, and community partners.
  • Valid driver's license and ability to meet Impact Life driving requirements.
  • Ability to travel throughout Sussex County as required to perform the essential functions of the position.
Preferred Qualifications
  • Experience in community outreach, harm reduction, behavioral health, substance use disorder services, recovery support, care navigation, case management, public health, or related human services.
  • Experience engaging individuals experiencing barriers to healthcare or treatment.
  • Knowledge of Sussex County treatment, recovery, healthcare, housing, workforce, and community resources.
  • Experience conducting screenings, making referrals, providing warm handoffs, or supporting linkage to treatment and recovery services.
