To call discharged Emergency Department patients at home to follow up on their care.

Responsibilities
- 1. Responsible for using EPIC reports to identify ED patients that were discharged .
- 2. Responsible for calling discharged patients to follow up on their visit and provide any necessary clarification of discharge instructions.
- 3. Provides service recovery as needed.
- 4. Calls to be made within 48 hours of patient discharge.
Qualifications
EDUCATION
Graduate of an accredited school of nursing (Diploma, AD, BS, BSN)
EXPERIENCE
2 years ED experience
LICENSURE
RN In State of CT or Permit; Basic Life Support (BLS)
SPECIAL SKILLS
Computer skills, strong service excellence telephone skills
PHYSICAL DEMAND
May be exposed to infectious diseases. Moderate stress. Light Duty.
