SUMMARY:

Works cooperatively with other team members to effectively implement the Hospice Plan of Care for patients.

 

REPORTS TO:

Home Care Clinical Director

QUALIFICATIONS:

  • Must be empathetic and responsible, able to use time effectively, be able to accept supervision, relate well to patients and understand and accept the hospice philosophy.
  • Current South Carolina Nurse Aide Certification.
  • Demonstrate competency in skills needed to perform job duties.
  • Ability to function as part of a team
  • Ability to embrace the mission and culture of the organization.

RESPONSIBILITIES:

  • Assist patients with baths, care of mouth, skin and hair.
  • Assist patients with elimination functions.
  • Help patients in and out of bed; assisting with ambulation.
  • Perform such incidental household services as are essential to patient health care.
  • Report any changes or concerns regarding patients' conditions and/or family situations to the registered nurse.
  • Maintain effective working relations with patients, families and members of hospice interdisciplinary team.
  • Perform other related duties as assigned.
  • Document all aspects of care provided according to the policies established for the Hospice/HomeCare program and the individual patients care plan.
  • Participate in organization sponsored events
