Summary:
The Licensed Practical Nurse (LPN) contributes to whole person healthcare by providing evidenced-based nursing skills to care for patients in both the Withdrawal Management and Intensive units. Â
Essential Duties & Responsibilities:
- Timely administer medications to patients and monitor patients for reactions or side effects Â
- Maintain accurate, detailed reports and records Â
- Record patients' medical information and vital signs. Â
- Monitor, record, and report symptoms or changes in patients' conditions. Â
- Receive and transcribe written orders from the medical and psychiatric providerÂ
- Medication Reconciliation process Â
- Facilitate effective crisis interventionÂ
- Assist the RN in consulting and coordinating with healthcare team members to assess, plan, implement, or evaluate patient care plans to include referrals to other levels of care when appropriate Â
- Assist the RN in modification of the patient treatment plans as indicated by patients' responses and conditions and collaborate within a multidisciplinary team to achieve best patient outcomes Â
- Direct or supervise residential technicians while on shift Â
- Provide medical re-education to patients as neededÂ
- Performs related duties as required.
*All agency staff are required to attend all mandatory department/agency meetings and trainings*
Schedule:
1st Shift: Â 7:00 AM - 730 PMÂ
Week 1: Monday, Tuesday, Saturday
Week 2: Sunday, Wednesday, ThursdayÂ

- Current Connecticut LPN licenseÂ
- Basic Life Support (BLS)/ CPR certification Â
