Job Summary:

CommunityCare Hazleton is seeking a Family Medicine or Med-Peds trained physician to care for patients of all ages at its growing primary care clinic in Hazle Township, PA.

Job Duties:

Job Description

CommunityCare, a Federally Qualified Health Center Look-Alike affiliated with Geisinger and qualifies for the National Health Service Corp Loan Repayment Program and the Pennsylvania Loan Repayment Program.

In this role, you will partner with interdisciplinary teams to manage population health, improve outcomes, and drive preventive care initiatives. The patient panel includes comprehensive care across the full lifespan, from newborns to end-of-life care.

Practice Overview and Schedule

  • 100% outpatient practice

  • 40-hour work week, including:

    • 36 patient contact hours

    • 4 administrative hours

  • Family Practice clinic hours (subject to change):

    • Monday - Thursday: 8:00 AM - 7:00 PM

    • Friday: 8:00 AM - 5:00 PM

    • Saturday: 8:00 AM - 4:00 PM rotation

  • One evening shift per week required

  • Shared call and weekend rotation required (1 in 12 – subject to change)

  • Onsite services: ExpressCare (urgent care), Laboratory, Optometry/Eyewear Center, Pediatrics, X-ray, Ultrasound, Behavioral Health

Patient Care and Clinical Expectations

  • Practice full-spectrum care for all ages

  • 20- and 40-minute appointments, determined by acuity

  • Collaborative care model with APP panel sharing (APP supervision required)

Technology and Documentation

  • Epic EMR

  • Abridge ambient voice dictation tool

Support and Resources

  • UpToDate access

  • Value-based care guides

  • Medication Therapy Disease Management on-site support

  • Behavioral health collaboration

  • 24/7 nurse call center

  • Centralized prescription refill team

  • Coding education support

  • Quality data monitoring (Provider Scorecard)

Compensation and Benefits

  • Compensation – salary ranges from $320k to $375k based on experience

  • Recruitment incentives up to $250,000, including residency/fellowship stipend (not applicable for internal, lateral transfers)

  • Continuing medical education – 15 working days and $4,500 CME funds

  • Paid relocation (not applicable for internal, lateral transfers)

  • Retirement plans - 401k, 403(b), and 457(b)
