The APRN may receive operational or clinical task assignments from authorized Kanza leadership and, when working within Health Department settings, will coordinate with designated Health Department personnel consistent with the collaborative service arrangement. Such coordination does not alter the APRN's employment relationship with Kanza or Kanza's responsibility for employment, clinical oversight, credentialing, performance management, and compliance. Classification: Exempt (Salaried) Employment Status: Full-Time This position is supported initially through a collaborative grant partnership between Northeast Kansas Multi-County Health Department and Kanza Mental Health and Guidance Center. The position is intended to be a sustainable Kanza Mental Health and Guidance Center position and will remain a Kanza employee regardless of the availability or continuation of grant funding. If grant funding ends or the collaborative service model changes, the APRN's responsibilities, service locations, patient population, and assignment of duties may be modified to meet Kanza's ongoing clinical, operational, and organizational needs. Position Purpose The Advanced Practice Registered Nurse (APRN) provides comprehensive, evidence-based, trauma-informed, and person-centered healthcare services through Kanza Mental Health and Guidance Center's Medical Services Department, with an initial emphasis on delivering care in partnership with and at local Northeast Kansas Multi-County Health Department offices. The APRN functions as an advanced practice clinician providing assessment, diagnosis, treatment, medication management, health education, preventive care, care coordination, and follow-up services within the APRN's education, national certification, population focus, Kansas licensure, demonstrated competency, clinical privileges, organizational policies, and applicable federal and state law. The APRN will provide and coordinate primary and preventive healthcare services with an emphasis on maternal and reproductive health, including prenatal and postpartum care; substance-use disorder treatment and medication-assisted treatment (MAT), including medication for opioid use disorder (MOUD); behavioral-health integration; chronic disease management; and other medical and community health services identified by Kanza Mental Health and Guidance Center. The APRN will provide care to individuals across the lifespan, as appropriate to the APRN's population focus and scope of practice, with particular emphasis on individuals and families who may experience barriers to healthcare access, including individuals with mental health conditions, substance-use disorders, co-occurring conditions, limited access to primary care, and other social or economic barriers to health. The APRN will collaborate with registered nurses, medical assistants, behavioral-health professionals, peer support staff, community health workers, pharmacies, hospitals, specialists, primary care providers, social-service agencies, Northeast Kansas Multi-County Health Department staff, and other community partners to promote continuity of care and improve health outcomes. The APRN is responsible for practicing within the scope of the APRN's specific role and population focus and in accordance with Kansas licensure requirements, applicable federal and state laws and regulations, professional standards, Kanza policies, clinical privileges, and established clinical protocols. Controlled-substance prescribing and MAT/MOUD services must be provided in accordance with applicable federal and Kansas requirements. VI. Care Coordination and Referral Management Coordinate care among individuals receiving services, family members or designated supports, nurses, medical assistants, behavioral-health providers, pharmacies, laboratories, hospitals, specialists, primary care providers, Northeast Kansas Multi-County Health Department personnel, and community organizations. Review outside medical records, laboratory results, hospital records, medication lists, and other relevant clinical information. Make appropriate referrals to primary care, specialty care, obstetric services, maternal-fetal medicine, behavioral health, substance-use treatment, emergency services, social services, and other resources. Follow up on referrals, diagnostic studies, laboratory results, hospitalizations, emergency department visits, and other significant clinical events. Assist individuals in navigating healthcare systems and accessing needed medical, behavioral-health, social-service, and community resources. Develop and maintain collaborative relationships with local hospitals, pharmacies, healthcare providers, behavioral-health organizations, Northeast Kansas Multi-County Health Department, and other community partners. Participate in multidisciplinary case consultation and care planning. Promote continuity of care when individuals transition between Kanza Mental Health and Guidance Center, the Health Department, hospitals, primary care, specialty care, behavioral health, and community services. VII. Clinical Documentation and Medical Records Accurately and promptly document assessments, diagnoses, treatment plans, prescriptions, orders, procedures, referrals, communications, patient education, and follow-up activities in the electronic health record designated by Kanza. Complete clinical documentation in accordance with Kanza requirements, professional standards, payer requirements, applicable laws and regulations, and any additional documentation requirements associated with collaborative programs. Review laboratory and diagnostic results and document appropriate clinical follow-up. Maintain accurate medication lists and complete medication reconciliation as appropriate. Document MAT/MOUD services in accordance with applicable clinical, regulatory, payer, and organizational requirements. Maintain appropriate documentation related to prenatal and postpartum care, referrals, screenings, and follow-up. Complete required reports, forms, quality measures, program documentation, and other clinical or administrative documentation within established deadlines. Maintain confidentiality and protect individually identifiable health information in accordance with HIPAA, 42 CFR Part 2, Kanza policies, and applicable federal and state requirements. VIII. Public Health and Community Health Responsibilities Participate in public health programs and initiatives consistent with Kanza's mission and collaborative agreements with Northeast Kansas Multi-County Health Department. Participate in communicable disease prevention, surveillance, screening, education, and response activities as assigned and within the APRN's scope of practice. Support community-based maternal and child health initiatives. Participate in vaccination clinics and other public health outreach activities as assigned. Support community education regarding maternal health, substance-use disorders, overdose prevention, behavioral health, chronic disease prevention, and other public health priorities. Participate in public health preparedness, emergency response activities, disaster response operations, and community health emergencies as assigned. Represent Kanza Mental Health and Guidance Center professionally in community meetings, outreach events, coalition activities, and collaborative initiatives, including activities conducted in partnership with Northeast Kansas Multi-County Health Department. Assist with program development, implementation, evaluation, and quality-improvement activities related to assigned clinical and community health services. IX. Team and Organizational Responsibilities Provide respectful, trauma-informed, person-centered, culturally responsive, and equitable healthcare services. Maintain professional boundaries and communicate respectfully with individuals receiving services, family members, coworkers, providers, and community partners. Work effectively as a member of an interdisciplinary healthcare team. Participate in staff meetings, clinical consultation, supervision, training, peer review, quality-improvement activities, credentialing activities, and organizational initiatives as required by Kanza. Provide clinical consultation, education, and guidance to nursing and medical support staff within the APRN's role, scope of practice, clinical privileges, and organizational structure. Assist in the development and review of clinical protocols, standing orders, workflows, patient education materials, and quality-improvement processes. Maintain current licensure, certification, continuing education, clinical competencies, and other professional requirements necessary for the position. Follow all Kanza policies related to safety, confidentiality, ethical conduct, technology, documentation, medication management, controlled substances, infection prevention, and professional behavior. Maintain dependable attendance and report to work as scheduled. Participate in public health preparedness, emergency response, vaccination clinics, community health initiatives, and disaster response operations as assigned. Perform other related duties as assigned by Kanza leadership. QualificationsMinimum Qualifications Master's degree or higher in nursing from an accredited program appropriate to the APRN's role and population focus. Current, unrestricted Registered Nurse (RN) license and Advanced Practice Registered Nurse (APRN) license in the State of Kansas, or eligibility for Kansas licensure prior to the start of employment. Current national board certification appropriate to the APRN's role and population focus from a nationally recognized certifying organization. Current certification in Basic Life Support (BLS) for healthcare providers. Eligibility to provide the clinical services required by this position within the APRN's education, certification, licensure, population focus, and demonstrated competency. Current prescriptive authority consistent with Kansas licensure requirements and the responsibilities of the position. If prescribing controlled substances, must maintain all required federal and state registrations and comply with applicable controlled-substance requirements, including an active Drug Enforcement Administration (DEA) registration when required for the position. Demonstrated clinical competence in assessment, diagnosis, treatment planning, medication management, patient education, and care coordination. Demonstrated ability to work with individuals experiencing medical, mental health, substance-use, or co-occurring conditions. Strong written and verbal communication skills. Ability to maintain confidentiality and appropriately manage sensitive medical, behavioral-health, and substance-use information. Ability to work independently while functioning effectively as part of an interdisciplinary team. Must successfully complete all required pre-employment screenings, background checks, and drug testing. Must possess a valid driver's license, maintain an acceptable driving record, and have reliable transportation to travel between Kanza and partner organization locations as needed. A Kanza vehicle may be available for required work-related travel between service locations. Must maintain all professional licenses, certifications, registrations, and continuing education required for the position. Preferred Qualifications Board certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Women's Health Nurse Practitioner (WHNP), or other APRN certification appropriate to the services provided by Kanza Mental Health and Guidance Center and its community health partners. Experience providing prenatal and postpartum care. Experience in maternal and reproductive health, women's health, family planning, or community maternal health. Experience providing medication-assisted treatment (MAT) or medication for opioid use disorder (MOUD). Experience treating individuals with substance-use disorders and co-occurring mental health conditions. Experience in community health, public health, federally qualified health centers, rural healthcare, integrated care, behavioral health, or other safety-net healthcare settings. Experience working with pregnant or postpartum individuals experiencing substance-use disorders. Experience collaborating with hospitals, obstetric providers, behavioral-health providers, pharmacies, public health agencies, and community-based organizations. Experience with electronic health records and healthcare practice-management systems. Experience with quality improvement, population health, grant-funded programs, or public health reporting. Bilingual or multilingual communication skills are preferred but not required. Working Conditions This position is an employee of Kanza Mental Health and Guidance Center and is initially assigned primarily to Northeast Kansas Multi-County Health Department office settings in Atchison, Brown, and Jackson counties as part of the collaborative medical services model. The position is initially supported through a collaborative grant partnership between Kanza Mental Health and Guidance Center and Northeast Kansas Multi-County Health Department. Grant funding is not a condition of continued employment. If grant funding ends or the collaborative service model changes, Kanza may modify the APRN's service locations, patient population, clinical assignments, and other responsibilities to meet Kanza's ongoing organizational and clinical needs. The APRN's primary administrative, employment, credentialing, and clinical accountability will remain with Kanza Mental Health and Guidance Center. When providing services in Health Department locations, the APRN will coordinate with designated Health Department personnel regarding site operations, scheduling, facility procedures, emergency protocols, and other matters necessary for effective collaborative service delivery. The employee may have contact with individuals experiencing physical illness, pregnancy-related complications, emotional distress, behavioral-health symptoms, substance-use disorders, or crisis situations. The position may involve exposure to blood, bodily fluids, infectious conditions, medications, controlled substances, cleaning products, and other healthcare-related hazards. Required safety procedures and personal protective equipment must be used. The APRN may be required to participate in community outreach, health fairs, vaccination clinics, home or community-based services, emergency preparedness activities, and other off-site activities consistent with the mission and operational needs of Kanza Mental Health and Guidance Center. Work Location Northeast Kansas Multi-County Health Department — Atchison, Brown, and Jackson County Offices, with ability to travel between offices and other community locations as needed. A Kanza vehicle may be provided for required work-related travel between service locations, subject to Kanza policy and vehicle availability.
