WMCHC’s Mission Wayne Memorial Community Health Centers creates and sustains healthy rural communities by providing accessible, high quality medical, dental, and behavioral health care for all, while recognizing and addressing other factors that influence health outcomes.  Responsibilities: The CHC Care Manager supports patients with low-to-moderate complexity needs by collecting structured patient information, identifying social and behavioral barriers, assisting with referrals, and coordinating follow-up care. The role focuses on improving access, navigation, and patient engagement by connecting individuals to needed services, supporting care-plan implementation, and collaborating with providers, RN Care Managers, and community resources. The CHC Care Manager plays an essential role in supporting population-health workflows and helping patients progress toward their care goals.   QualificationsQualifications: Graduate from an accredited nursing program (LPN) with current licensure  Demonstrates high degree of clinical and leadership competence Minimum of three recent years in healthcare organization or social services field Required Skills/Abilities: Strong clinical assessment skills with ability to manage complex medical conditions Expertise in evidence-based guidelines, chronic disease management, and patient education Highly developed interpersonal skills for collaboration across the care team Demonstrated ability to lead care planning, critical thinking, and clinical decision-making Ability to form therapeutic, longitudinal patient relationships with appropriate boundaries Intermediate computer skills with willingness to acquire advanced population-health platform skills Ability to work independently and serve as a clinical resource to team members Strong organizational skills with ability to manage multiple high-acuity caseloads
