Job Role Summary

The Social Worker – Gender Health is responsible for providing comprehensive social work services which assist providers, patients and their families to find practical ways of overcoming social, emotional, and environmental barriers to optimum functioning and attainment of health. This position acts as educator for clients, families, community and other professionals regarding disease prevention, impact of illness and disease progression, advocacy for benefits, health maintenance, and adherence to treatment regimens.

Job Requirements

•    Graduation with a Master’s Degree from an accredited school of social work; must be licensed or become licensed as an LSW 
•    Social work experience in a health care setting dealing with adolescent, adult and geriatric patients is preferred
•    Case management and/or care coordination experience preferred
 

Essential Functions and Responsibilities

•    Responsible for providing comprehensive social work services which assist providers, patients and their families to find practical ways of overcoming social, emotional, and environmental barriers to optimum functioning and attainment of health.
•    Provides social work intervention for clients ranging from short-term counseling, crisis intervention, community resource referral, client advocacy, support, outreach, follow-up and intra/inter-agency consultations
•    Provides ongoing assessment, including gathering comprehensive information to use in developing interventions and treatment strategies; included are psychosocial assessments and referrals 
•    Implements intervention and treatment plans that promote client well-being and ensure a continuum of care.  Planning is based on a comprehensive, culturally competent assessment with interdisciplinary input and includes:
o    Basic needs-referrals to community resources for food, clothes, housing/shelter, utilities, etc.
o    Access to medical care – transportation, medication copays, Medicaid, Medicare Parts A, B and D, QMB and Extra Help
o    Home health care, including referrals to CICOA for in-home services, durable medical equipment ordering, and caregiver support and assistance
o    Placement – assisted living, nursing home, group home
o    Abuse/neglect – assist and refer domestic violence victims, APS and CPS reports, referral to resources for prevention of abuse
o    Legal assistance that includes Medical Legal Partnership referrals, legal aid and notary services
o    Education and employment –referral to GED programs, literacy and high school diploma programs, referrals to vocational rehabilitation and employment search programs
o    Financial – Budgeting assistance, SSI/SSDI information, etc.Case management – coordinate culturally sensitive care, goal-setting with clients, and reassessment of progress toward goals
•    Acts as educator for clients, families, community and other professionals regarding gender diversity, gender identity development, advocacy for benefits, health maintenance, and adherence to treatment regimens
•    Advocates on behalf of clients, evaluates their needs and assists them with getting connected to services
•    Initiates advocacy/liaison role for clients in referrals to and from community agencies; collaborates with agency and hospital personnel in referral activities, and cultivates referral resources
•    Maintains records or documentation of social work services, which reflect the client and client systems’ pertinent information for assessment and treatment; social work involvement and outcomes with and for clients; and in accordance with care goals and legislative and administrative regulations and policies
•    Participates in staff meetings, case conferences, and continuous professional self-development
