About the Role:
Unlimited Abilities is seeking a compassionate and organized Service Coordinator to support adults receiving Home and Community-Based Services (HCBS) and Habilitation (HAB) services. This role serves as the primary point of contact for members, families, and stakeholders while ensuring compliance with waiver regulations and delivering high-quality, person-centered support.

Key Responsibilities:
  • Coordinate client admissions, assessments, intakes, and onboarding.
  • Develop, update, and maintain Individual Service Plans (ISPs) and client records.
  • Conduct monthly face-to-face visits with individuals receiving services.
  • Collaborate with House Leads, care teams, and community partners to ensure services are effectively implemented.
  • Assist with referrals to community resources and support services.
  • Review and follow up on incident reports and quality assurance recommendations.
  • Maintain accurate documentation and ensure compliance with all state, federal, and agency requirements.
  • Serve as a liaison between clients, families, providers, and other stakeholders.
  • Participate in meetings, trainings, and ongoing quality improvement efforts.
Qualifications:
  • Strong organizational and communication skills.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Experience in human services, case management, social work, healthcare, or a related field preferred.
  • Commitment to person-centered care and supporting individuals in achieving their goals.
