Description

Iowa City Hospice is growing and seeking a compassionate and dedicated Part-Time Social Worker to join our team.  This role offers an excellent opportunity to make a meaningful impact in the lives of individuals and families while maintaining a flexible schedule. We take pride in our exceptional patient-centered hospice services provided to patients and families in their own home, regardless of disease, age, background or ability to pay.  As a Hospice Social Worker, you will be responsible for providing providing emotional support, counseling, and community resources to patients and their families from time of admission until end of services; then assessing the bereavement needs of the patient’s survivors. 


This position requires exceptional communication skills, strong attention to detail, adaptability, and the ability to successfully manage responsibilities in an ever-changing work environment. Candidates must demonstrate professionalism, initiative, and consistent follow-through while working collaboratively with patients, families, community partners, and healthcare professionals.


Key responsibilities include conducting comprehensive psychosocial assessments to identify the emotional, social, financial, and support needs of patients and their families; developing and implementing individualized care plans; providing counseling, guidance, and emotional support to help patients and families cope with illness and end-of-life challenges; and connecting individuals with appropriate community resources, support services, and support groups.


The role also involves advocating for patients and families, monitoring and evaluating patient progress, maintaining accurate and confidential documentation, and collaborating with interdisciplinary healthcare teams to ensure comprehensive, patient-centered care. Participation in team meetings and ongoing care planning discussions is expected to support quality outcomes and continuity of care.

Requirements

  • Bachelor's Degree in Social Work from a CSWE-accredited program required; Master's Degree preferred.
  • Current Iowa LMSW, LISW, or LBSW licensure preferred.
  • Minimum one year of medical social work, counseling, or related healthcare experience preferred.
  • Full-time availability, Monday through Friday, 8:00 AM to 5:00 PM.
  • Valid driver's license and current automobile insurance required.
  • Knowledge of medical terminology, healthcare systems, community resources, and social service programs.
  • Strong communication, interpersonal, organizational, and time-management skills.
  • Ability to work independently while collaborating effectively as part of an interdisciplinary team.
  • Experience conducting psychosocial assessments, care planning, counseling, and patient advocacy.
  • Compassionate, empathetic, and patient-centered approach to care.
  • Ability to connect patients and families with appropriate community resources and support services.
  • Demonstrated professionalism, sound judgment, and commitment to maintaining accurate and confidential documentation.
