The RoleĀ 

As a Discharge Transportation Coordinator, you manage and coordinate patient transportation services across UVA Health using a variety of resources, including ambulances, wheelchair vans, taxis, public transportation, and other community-based options.Ā 

Working from UVA Health Town Center One, you serve asĀ a central pointĀ of contact for transportation planning, balancing patient needs, insurance requirements, transportation availability, and operational priorities to ensure efficient patient movement.Ā 

Your work includes:Ā 

  • CoordinatingĀ patient discharge transportationĀ using ambulances, wheelchair vans, taxis, buses, and other transportation resourcesĀ 

  • Communicating withĀ patients, families, clinical teams, case managers, and transportation vendorsĀ to arrangeĀ appropriate transportĀ solutionsĀ 

  • Navigating insurance coverage requirements and transportation eligibility guidelines to support efficient discharge planningĀ 

  • Prioritizing transportation requests while managing multiple cases simultaneously in a dynamic healthcare environmentĀ 

  • Investigating transportation concerns, resolving service issues, and supporting positive patient experiencesĀ 

  • Tracking transportation activity, processing invoices, andĀ maintainingĀ accurateĀ recordsĀ in accordance withĀ departmental proceduresĀ 

  • Supporting process improvement initiatives and assisting with trainingĀ and onboarding of new team membersĀ

Education: High school diploma/GED

Experience: Relevant experience in the field.

Licensure

PHYSICAL DEMANDS

This is primarily a sedentary job involving extensive use of desktop computers. The job does occasionally require traveling some distance to attend meetings, and programs.

The pay range for this role is $20.00 - $24.73 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
