Registered Nurse Care Coordinator (RNCC) - Pulmonary Clinic, Children's Hospital

As a Registered Nurse Care Coordinator in the Pediatric Pulmonary Clinic, you will guide patients and families across the care continuum through telephone triage, education, treatment support, and coordination with specialty services. This is an opportunity for an experienced nurse to practice with autonomy, flexibility, and meaningful interdisciplinary partnership while becoming a trusted clinical resource for a defined pulmonary patient population. 

The Role 

This position blends clinical coordination with the critical behind-the-scenes work that keeps complex pediatric care moving forward. You will collaborate closely with pulmonary providers, respiratory therapists, clinical team members, and fellow RN Care Coordinators to anticipate needs, remove barriers, and help families understand and navigate their care plans. 

Your average day may include: 

• Providing telephone triage and using nursing assessment skills to identify needs, establish priorities, and support appropriate next steps 

• Coordinating care between Pediatric Pulmonary and other services involved in each patient’s treatment 

• Delivering clear, individualized education to patients and families regarding pulmonary conditions, treatments, and care plans 

• Supporting clinic visits, patient teaching, and pulmonary treatments 

• Managing forms, prior authorizations, and other administrative requirements that help patients access timely care 

• Communicating with providers, respiratory therapists, nurses, and clinical partners to maintain continuity across settings 

• Applying critical thinking and problem-solving skills to address clinical and operational barriers 

• Progressing toward independent management of the pulmonary patient population 

The role is primarily focused on care coordination and triage rather than continuous in-person bedside care. It is well suited for a confident nurse who values clinical ownership, organized follow-through, and the opportunity to make an impact across the full patient journey. 

This is a salaried position with a Monday through Friday, 8 a.m. to 5 p.m. schedule in a clinic and office environment. Some remote work may be available after onboarding, providing added flexibility as you become established in the role.
