Registered Nurse Patient Navigator

Location: Jacksonville, FL 

Facility: Ascension St. Vincent's Riverside Hospital 

Schedule: Day shift | Full-time

How you'll make an impact in this role

• Directs patients seamlessly through every stage of treatment and follow-up care as a dedicated navigator

• Connects administrative staff, physicians, managed care payers, and community partners as a key central liaison

• Drives coordinated patient treatment plans while elevating ongoing clinical education and quality improvement efforts

• Tracks outcomes and analyzes key metrics to recommend targeted process and care improvements

• Secures critical follow-up care referrals and essential community resources for uninsured and underinsured patients

What minimum qualifications you'll need

Licensure / Certification / Registration:

• Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or jobtransfer date required.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time ofhire.
