Job Summary

GetWell Health System is seeking a compassionate and organized Targeted Case Manager (TCM) to support individuals experiencing behavioral health and substance use challenges. The Targeted Case Manager will coordinate care, connect clients with community resources, and assist patients in achieving improved mental health stability, wellness, and independence.

The ideal candidate is passionate about helping underserved populations, skilled in care coordination, and experienced in behavioral health services, Medicaid documentation, and community-based support systems.

Key Responsibilities

  • Conduct behavioral health assessments and identify client needs.
  • Develop individualized treatment and service plans in collaboration with providers and patients.
  • Coordinate referrals for psychiatric services, therapy, housing assistance, transportation, employment, and social services.
  • Maintain ongoing communication with patients, families, healthcare providers, and community agencies.
  • Monitor patient progress and update care plans as needed.
  • Assist clients with crisis intervention and linkage to emergency services when appropriate.
  • Educate patients about available behavioral health and recovery resources.
  • Ensure all documentation is completed accurately and timely in compliance with Kentucky Medicaid and company standards.
  • Participate in interdisciplinary treatment team meetings.
  • Support continuity of care during hospitalizations, discharge planning, and transitions between levels of care.
  • Maintain confidentiality and comply with HIPAA regulations.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, or related field required.
  • Experience in behavioral health, substance abuse treatment, or case management preferred.
  • Knowledge of Kentucky Medicaid Targeted Case Management guidelines preferred.
  • Strong communication, organizational, and documentation skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Basic computer and electronic health record (EHR) proficiency.
  • Valid driver’s license and reliable transportation may be required.

Preferred Skills

  • Crisis management and de-escalation skills.
  • Knowledge of community behavioral health resources in Kentucky.
  • Experience working with individuals with co-occurring disorders.
  • Cultural sensitivity and trauma-informed care approach.
  • Ability to manage multiple cases effectively.
