As a Nurse Care Manager, you’ll be at the heart of client wellness, supporting individuals with chronic conditions and behavioral health needs. What You’ll Do Collaborate with an interdisciplinary team to develop client-centered wellness plans Conduct health and metabolic screenings (quarterly or annually) Provide outreach calls to engage and support clients Lead health and wellness education sessions and group activities Monitor hospital admissions/discharges and coordinate medication reconciliation Partner with healthcare providers to ensure continuity of care Track and assess performance measures related to physical health Occasionally (up to 25%) meet clients in the community or in their homes Requirements of the Position Graduation from an accredited nursing program Active, unrestricted Missouri LPN or RN license Valid driver’s license, proof of insurance, and a reliable vehicle (Preferred) 2+ years of nursing experience in a mental health setting Familiarity with mental illness and substance use disorders Strong communication and customer service skills Comfort with electronic health records and intermediate computer proficiency (Microsoft Office and use of email) Grow With Us We’re committed to your professional development and will support your growth as a Nurse Care Manager through training, collaboration, and meaningful work.
