What You’ll Do You’ll play a vital role in supporting families through education, guidance, and connection: Conduct home visits with families (prenatal through kindergarten) Provide parenting education and child development information Develop individualized plans and set goals with families Use screening tools (ASQ-3, ASQ-SE, Edinburgh, etc.) and make referrals Document visits and maintain accurate records in required systems Coordinate and facilitate group connections (10–12 annually) Build relationships with community partners and service providers Maintain consistent communication and follow-up with families Manage a caseload based on program requirements What You Bring A passion for supporting families and strengthening communities. Experience: Two years of supervised experience with young children or parents is preferred but not necessary. Core competencies Required:  Exceptional organizational skills; proficient computer skills; interpersonal relationship skills; the ability to communicate orally and in writing with the ability to work effectively in a fast-paced environment; manage several projects simultaneously; and to adjust to frequently changing demands. Possess communication skills in order to verbalize the needs of troubled youth and the agency’s philosophy to the public and be able to communicate and work effectively in a professional manner with other social work personnel. Be able to communicate and work effectively with agency personnel and community and state agencies in a productive and professional manner. Ability to respond to and manage crisis situations. Must have a valid state issued driver’s license and submit to an intensive background investigation. Must be willing to adapt and learn court proceeding processes. Must be self-motivated and task oriented. Must be comfortable speaking in front of small groups. Must be willing to travel on occasions. Must attend ALL required PAT Training.  Preferred:    NA Certifications: NA Licenses: Driver’s license required
