Job Purpose:
We are seeking a dedicated Clinical Liaison/Admissions Coordinator to coordinate patient admissions, ensure seamless communication between healthcare providers, and advocate for patient needs. The ideal candidate will have a solid healthcare background, strong relationship-building skills, and the ability to navigate complex systems to deliver high-quality, patient-centered care

Essential Duties and Responsibilities:

- Manage and direct the admissions process with referrals which include hospitals, Doctors’ offices, nursing homes, and other related businesses according to established policies and procedures.
- Screen and accept referrals timely with the understanding that outliers are to be reviewed by the Administrator/DON.
- Obtain Insurance Authorization when necessary
- Complete all necessary admission paperwork including daily communication protocols
- Coordinate with nursing and housekeeping staff to update bed availability, and ensure rooms are clean, labeled, and ready for potential admissions.
- Ensure all admissions-related documents are uploaded into the medical records system.
- Maintain updated bed availability and daily census.
- Communicate special needs of new admissions to staff ensuring a smooth transition.
- Maintain knowledge of Medicare and Medicaid regulations.
- Conducts tours to effectively present the center and convert prospective residents.
- Promotes a high standard of customer service and satisfaction, including staff education, service improvement initiatives, and a commitment to person-centered care.
- Adheres to all company policies, procedures, and standards of conduct, reporting any concerns as appropriate.
- Performs additional duties as assigned.

Position Requirements:
- Marketing and sales experience preferred. Strong written and verbal communication skills, with the ability to deliver effective presentations.
- Demonstrated ability to build professional relationships and convert inquiries into customer relationships through effective communication, including phone-based outreach.
- Proficient in Microsoft Office (Word, Excel, PowerPoint) and able to quickly learn specialized systems and applications.
- Working knowledge of healthcare reimbursement programs preferred.
- Strong organizational, time management, and documentation skills, with the ability to prioritize and manage multiple tasks effectively.
- Ability to handle sensitive information with confidentiality and professionalism.
- Self-motivated and goal-oriented, with the ability to work both independently and collaboratively.
- Flexible and adaptable schedule to meet the demands of admissions and business needs.
