Facilitates and coordinates hospital-wide discharge planning and support programs to ensure compliance with Joint Commission and other health care accrediting standards in order to enhance the quality of patient care consistent with hospital policies for the adult and geriatric patients, 18 years and over. QualificationsEducation: Master level Social Worker degree. Preferred Master level social worker/advanced clinical practitioner. LICSW or LGSW preferred Licenses/Certification: Current certification issued in the state of facility location. Experience: Minimum of three (3) years acute care experience to include experience in discharge planning procedures. Medicare/Medicaid guidelines/knowledge of community resources Safety Sensitive-Designated Position
