Title: Health Care Companion
Schedule: Monday - Friday, 8:00am - 5:00pm
Non-Certified Caregiver Pay: $18.31 to $22.59 per hour
Certified CNA Pay: $23.29 to $25.40 per hour

JOB SUMMARY

The Health Care Companion works in collaboration with the transportation team to accompany participants who need escorts during transportation and/or outside appointments. The job may include assisting with transfers, paperwork, and waiting with participants at appointments. When not escorting participants, they may be tasked with duties in the Day Center. PACE Health Care Companions will also assist with the organization and implementation of activity programs in the Day Center.

RESPONSIBILITIES

  • Assists transportation staff with the loading and unloading of participants from vehicles.
  • Accompanies enrollees to medical and other appointments as needed.
  • Assists participants with transfers, toileting, and other needs during transportation and at appointments.
  • Throughout the day, assists in the day rooms, kitchen, and personal care areas when not escorting.
  • Notes changes in participant’s physical or emotional health and communicate this information to the IDT.
  • Monitor health status of participants and provide a safe environment.


EDUCATION/EXPERIENCE:

  • Regular high school degree or equivalent.
  • Experience in healthcare as a caregiver or CNA.
  • Experience working with individuals with mental health needs (Preferred).
  • Experience working with the frail or elderly population (Preferred).
  • Experience working within an Interdisciplinary Team Model.
  • Experience with Electronic Medical Records (EMR).
