Schedule: Monday - Friday 8am-4:30pm, back-up on call rotation, holiday on call rotation
Territory: Lafeyette IN area
SUMMARYÂ
A Registered Nurse (RN) I - Hospice provides and plans professional nursing care to patients under the direction of a physician. This role involves delivering direct patient care, assisting with examinations, accurately documenting patient conditions, vital signs, diagnoses, and prescribed treatments. The Level 1 RN also administers treatments and medications as directed, contributing to the comfort and symptom control of hospice patients.Â
KEY RESPONSIBILITIESÂ
Provides skilled nursing care in a patient’s home as ordered by the attending physicianÂ
Observes and monitors patient conditionsÂ
Performs assessments to develop an individualized plan of care and adjusts as needs changeÂ
Administers medication as prescribed by the physicianÂ
Manages multi-disciplinary care as applicableÂ
Promotes continuity of care with appropriate admissions, transfers and dischargesÂ
Counsels’ patient and family on the disease/injury processes and how to manageÂ
Incorporates patient and family in development of plan of careÂ
Listens to patient and family membersÂ
Oversees and supervises total care of patient provided by Aides and LPNsÂ
Evaluates the treatments and medical condition of the patient according to the plan of careÂ
Other duties as assignedÂ
QUALIFICATIONSÂ
Performs work under minimal supervision. Handles complex issues and problems and refers only the most complex issues to higher-level staff.Â
Possesses comprehensive knowledge of subject matter.Â

EDUCATION AND CREDENTIALSÂ
Valid RN license in the state(s) where service is providedÂ
Bachelor's degree in nursing or healthcare-related field preferredÂ
