The Peer Navigator/Healthcare Navigator provides patient education, testing support, care coordination, and recovery-focused navigation for individuals receiving services through the clinic, mobile SSP unit, and community outreach settings. This position supports HIV, HCV, STI, and PrEP-related services; assists patients with insurance coverage, patient-assistance programs, appointments, referrals, and follow-up care; and helps link patients to infectious disease care, substance use disorder treatment, medication for opioid use disorder, mental health services, and other health and social-service supports. The role requires accurate documentation in the electronic health record, TN-WITS, and program logs, as well as participation in reporting, meetings, supervision, training, and related program activities. The ideal candidate has knowledge of harm reduction, recovery support, substance use disorders, and HIV/HCV/STI prevention and treatment, and is able to provide respectful, trauma-informed, nonjudgmental support while maintaining confidentiality, professional boundaries, and client self-determination.
