About the Role
Primary Care Partners is seeking a compassionate, reliable, and organized Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our clinical team as a Triage Nurse.
In this role, you will serve as an important connection between our patients and providers. You will assess patient concerns, provide telephone triage, coordinate follow-up care, communicate test results and provider recommendations, assist with medication refill requests, and ensure patients receive timely and appropriate care.
The ideal candidate will have strong clinical judgment, excellent communication skills, and the ability to manage multiple priorities in a fast-paced primary care environment.
Responsibilities Include:
Provide telephone triage and assess patient symptoms, concerns, and care needs.
Return patient calls and provide appropriate clinical guidance within scope of practice.
Communicate patient concerns, urgent needs, and changes in condition to providers as appropriate.
Provide timely patient education, medical advice, test results, and provider recommendations.
Assist with medication refill requests and maintain accurate medication documentation.
Coordinate follow-up care and assist patients with scheduling appointments when appropriate.
Identify urgent or emergent concerns and provide appropriate recommendations and escalation.
Ensure accurate, thorough, and timely documentation in the electronic health record.
Collaborate closely with providers, clinical staff, and other team members to ensure continuity of care.
Support overall clinic flow and contribute to a positive, team-oriented work environment.
Assist with additional clinical duties and responsibilities as needed.
Qualifications:
Completion of an accredited nursing program.
Current RN or LPN license in good standing.
Previous clinical, primary care, medical office, or telephone triage experience preferred.
Proficiency with electronic health record systems.
Strong clinical judgment, critical-thinking, and organizational skills.
Excellent communication and interpersonal skills.
Ability to work independently while effectively collaborating with providers and team members.
Ability to prioritize multiple tasks and remain organized in a fast-paced environment.
Job Details
Job Type: Full-Time, 36 hours per week
Pay: $27.00–$33.00 per hour, based on licensure and experience
Schedule: Day shift, Monday–Friday
